2. Bromley Learning Disability Partnership Board. We completed the Annual Health Self Assessment in June 2011. The Assessment looked at how health services in Bromley were meeting the needs of people with a Learning Disability in Bromley. We discussed the Assessment and received your feedback at the Partnership Group in May 2011

3. Bromley Learning Disability Partnership Board. We met with NHS London in August 2011, and they checked our Assessment and Scores. We had made some improvements since last year, but still have some work to do to make things better. We will be assessed again next year!

4. Bromley Learning Disability Partnership Board. In total there were 26 areas that we had to score against. Unfortunately we had 2 more RED  score’s in 2011. Well done, we also got 6 more Green score’s in 2011 SummaryLast Year 2010 This Year 2011 Red  24 Amber  2114 Green 28

5. Bromley Learning Disability Partnership Board. Where did we score Red  ? Campus Reprovision Annual Health Checks Access to Health Screening and Prevention Services Health Care provision for those in the Criminal Justice System

6. Bromley Learning Disability Partnership Board. Where did we score Green ? Community Health Services (e.g.Dental, District Nursing) Use of Electronic Information and Systems in Health Services Addressing learning points from previous investigations (e.g. six lives) Local Strategy for Respite Services Services to avoid unnecessary admission to hospital Transition Planning Partnership working between the PCT and Local Authority Mental Health Services for those with a Learning Disability

7. Bromley Learning Disability Partnership Board. We will be working together over the next few weeks to put an ‘action plan’ in place with services to improve things in the areas we scored Red and Amber. Good News – we will have a person working with us to help us take this forward. The Good Health Sub-Group Will monitor progress during the year and report back to you.
