Bring your child to the office. Complete an excuse slip. Provide a packed lunch.

Call the office to tell them what time your child must leave for the appointment.

Write a letter to the building principal at least one week before the trip. Include: Destination Dates of absence People traveling with child

Bring the item to the office. The secretary will give it to your child.

Borrow from the office Repay within 3 days

Contact your child’s teacher (due to our Wellness Policy, only certain items may be brought into school) Bring treat to office

Go to the office to obtain and sign a withdrawal form. The office will give you a copy of the following: Birth Certificate Immunization Social Security
