Primary Care Quality & Information Service (PCQIS) The purpose of the PCQI Service is to assist primary care practitioners to improve the quality of the service they deliver by providing access to evidence- based quality improvement guidance and tools

Primary Care Quality Forum  PCQF is the key component of the implementation of the PCQI Service  Forum will focus on delivery of Primary Care quality support tools to LHBs and practitioners and become a steering group for the PCQI Service  To agree a work programme for the service which all LHBs/practitioners wish to access

All Wales Clinical Governance Practice Self Assessment Tool  LHB Medical Directors commissioned PCQI Service to develop Clinical Governance Practice Self Assessment Tool  Initial meetings held in North and South Wales with PCQIS and Medical Director / CG Manager / GPC (Wales)  CG Tool based on Conwy/Denbighshire LHBs and IMH MAAG models  Microsoft Access Version developed in December 2005

The Aim  To improve the quality of care provided by primary care  To assure patients, practitioners and commissioners that effective CG procedures are in place in primary care practices

The Objectives  To produce an electronic tool: 1. That can be used by practices to review progress made in the development of CG processes within their practices 2. That can be used as the basis of assessment by LHBs of the level of CG activity within practices

The Objectives 3. That can be referred by inspecting bodies, such as HIW as evidence that appropriate CG activity is occurring in practices 4. That together with a visiting programme by LHBs can be an assurance to the GMC that GMPs are working in an environment where CG is operating effectively and standards of work of practices are being assessed externally

The Principles  Standardised model across Wales  Produced for practices to improve systems within their practice  Completed by practice team and key developments can by fed into Practice Development Plan

All Wales Clinical Governance Practice Self Assessment Tool (CGPSAT)  An e-questionnaire has been developed:  Incorporates advice, sources of further information and recommends suitable evidence  Information can be used to produce a practice development plan  Endorsed by GPC (Wales)  A web based version available in June  Incorporates questions on Child Protection and ISMS
