Identified Area for Improvement: Up-skill the rural primary care workforce by: Producing a skills and competencies framework for practice nurses, health care assistants and practice managers Delivering a programme of training to support the framework

Identified Area for Improvement: Establishing the role of Physician Associate across Mid Wales Introduce a 4-year PA development programme (more later) Roll out the concept of the ‘virtual ward’ to all parts of Mid Wales Agree the core principles of the ‘virtual ward’ and then allowing for local variation to reflect local circumstances, ensure the core principles are in place across the area

Identified Area for Improvement: Establish an integrated community focussed dental service in Mid Wales Develop an intermediate oral surgery service for complex extractions Utilising Community Dental Service staff, develop a joint GA list in Bronglais Hospital Develop a maxillofacial service which avoids the unnecessary use of secondary care services and patients travelling to South Wales

Identified Area for Improvement: Establish an enhanced community-focussed service which supports the timely discharge/ transfer of ‘medically fit’ patients from Bronglais Hospital Identify current ‘blockages’ and implementing service changes to over-come them

5 Community Services Primary Care & Progress to Date:
Collaboration with University of Birmingham established A longer-term commitment to Mid Wales is essential 4 year programme: 2 years in university, with clinical placements in Mid Wales (primary care and hospital) 2 years in paid employment (primary care and hospital) where clinical skills are further developed while in-service Funding partly in place; unconfirmed commitment for all partners received Aim is to support first intake of students from 2016
