Achievements APCC Collaboratives - a QAIHC/GPQ initiative 50,000 active patients in database 40,000 in AICCHS Hero Rewards – phase 1 and moving into phase 2 Growth in numbers of health checks undertaken across sector Continuation and expansion of Workforce Development across the sector Successful WSU establishment and operations AOD training – 50+ successful completions of courses over 2 years

Achievements Sector and new service development Regionalised support models Focus upon Recovery Plans and support Continuation and expansion of health promotion initiatives and activities – covering: healthy lifestyles, supportive environments, nutrition Expansion of healthy lifestyle initiatives across all of sector Moving in to mental health promotion – Voices United Choirs

Achievements Formation of new Partnership agreements – GPQ and QH Long-term agreements with major service delivery partners Accreditation compliance –Only state with 100% participation rate of members services in accreditation programs Formation and growth of BQC Commenced full operations as of 1 July 2011
