Orientation (September 30th) All mentors and students attend the orientation and receive more detailed information on the program along with time for each group to get to know each other 6 meetings (over 16 months, 2 hours twice a term) Topics include : Words and Meanings, Living with Chronic Disease/Disabilty, Experiences with the Health Care System Symposium (late April/early May) All participants gather to share and synthesize their learning through creative presentations

Students will be recruited by each participating program; Participating programs: audiology, dentistry, dietetics, genetic counseling, medicine, kinesiology, nursing, occupational therapy, pharmacy, physical therapy, speech-language pathology; Students are volunteers who: o receive academic credit as part of their program o receive 65 immersion level points towards IPE (Interprofessional Education) passport;

Learn about the experience of chronic disease, and the range of work involved in its management from the perspective of the patient and family Explore their roles as individual practitioners and interprofessional teams in supporting chronic disease self- management, addressing psychosocial as well as biomedical needs. Gain insight into new perspectives Hear experiences first-hand from an individual living with a chronic illness or disability Have the opportunity to discuss and learn with members from various health disciplines Document their learning over the course of the program through entries in a reflective journal
