(Primary care=General practice) Thurrock Primary Care (General practice) Achievements in the last 12 months Primary Care transformation (Primary care=General practice) Successfully secured £250k recurring over seven years from the Primary Care Transformation Fund to deliver weekend primary care services on a hub model across four localities. All four GP Health Hubs (Corringham, Stanford le Hope, Tilbury, Grays and South Ockendon) are now up and running. Embarked on a plan with NHS England and Health Education England to attract and retain doctors and nurses working in Thurrock. Benefitting from an Essex-wide Primary Care Workforce Career and Leadership Development Centre to increase the primary care workforce. General Practice workforce General Practice support and development A bid was submitted to secure funding to deploy a programme of work to support the development of General practices. The programme will: support practices to deliver high quality patient care. Reduce variation and share best practice. General Practice estates development A committee was established to complete an estates strategy to optimise the use of current buildings and facilities. This committee will: Undertake the planning and development of the new facilities in Tilbury and Purfleet. Review and identify future service provision for the new facilities in the medium and long term.

Thurrock Primary Care (General practice) 2015/16 future plans Over the last few years the General practice in Thurrock is under immense pressure from a shrinking work force and increased work load due to ageing population. To address this the CCG aims to undertake the following future development Further development of the locality GP Health Hubs by extending the level of service provision and also offering emergency appointments Integration of the GP Health Hubs with other hospital and community services with an aim to provide one stop clinics closer to homes Work collaboratively with all the partners to increase the General practice workforce by recruiting additional GPs and nurses in Thurrock. Develop a programme of work to train and up skill nurses, pharmacists and other health professionals to take on additional clinical responsibility. This is expected to free up GP time to attend to more complex patients. Provide a tailor made one to one support to general practices to reduce variation in service delivery Build newer purpose built integrated health and social care facilities in Tilbury and Purfleet with the aim of providing one stop services and improving overall patient care and outcomes.
