2 Where are we now? The practices are currently working in premises which are not fit for purpose and are at full capacity, this includes: One practice in a Victorian property – Kildonan House. One practice in a terraced house –Bolton Community Practice CIC. One practice in a 1970’s health Centre – Pike View. Combined, the practices currently serve a population of almost 25,000 patients. A further 1,700 homes planned in Horwich and nearby area, therefore an additional 5,000 residents and patients will need to be accounted for. This requires an improved quality and increased size facility to do so.

3 What are we planning? The three Horwich practices worked collaboratively to produce a bid to develop new primary care premises in February 2015. The Primary Care Infrastructure Fund bid was supported in principle by NHS England in March 2015. Plan is to develop a multi housed GP practice facility with Community and Social services also operating in the same building. The facility will be directly next to the new Horwich Leisure Centre, replacing the old 1960s leisure centre.

4 Future service model Provision of enhanced care closer to home for the elderly and those with chronic or complex needs. Embraces co-ordination of care and services – including well-being activities - across the breadth of health and social care and including voluntary organisations. Provision of accessible diagnostics and treatment facilities. Provision of a range of care and services outside traditional hours and over an extended week. Development of integrated training hub.

6 Progress to date? March 2016: Enablement agreed by NHS England to progress the design of the build. April 2016: Initial design developed by three Practices and feasibility studies commissioned. April 2017: Appointment of Turner & Townsend Project Management to engage with stakeholders to produce capacity modelling for healthcare facility planning. June 2017: Turner & Townsend Project Management further appointed to provide Project Management support, including appointment of a Design and Cost Management Team. September 2017: Turner & Townsend Project Management instructed to progress to Detailed Design which will allow full Cost Analysis and to send the design out for Local Planning Approval.

7 What is Capacity Modelling?
Capacity modelling allows for planning of the space within the building as to make sure the Stakeholders receive the most efficient building for their work pattern. Capacity modelling has been undertaken using current and anticipated activity levels. The capacity modelling allows for both Primary Care Services and for Community and Social Services, with engagement from the Practices, Commissioners and Bolton Council. The modelling has been undertaken with reference to the anticipated primary care workforce of the future which and also allows for the three practices to work together to gain the best use of the space provided in the building.

8 Capacity Modelling Results
The capacity modelling has highlighted the below requirements: Allowance for seven day working. Parking spaces for 153 cars. Increased training facilities for staff and local community providers. 39 x Consultant exam rooms for primary care delivery. 4 x Treatment rooms for primary care delivery. 2 x Consult exam rooms for provision of community services. 2 x Treatment rooms for provision of community services. 1 x Multifunctional group room for provision of primary care and community services. Allowance for mobile diagnostics suite in car park. Wifi access.

9 Development Outcomes Fit for the long term future needs of the population. Delivers a transformed model of care. Encourages an ‘asset’ and ‘place’ based approach to care. Leads to improved population health and well-being. Effective and efficient use of shared space for all stakeholders. Multi patient friendly building, easy to navigate. Multi-use rooms and areas with layout facilitating culture of multidisciplinary working. A facility that embraces the digitally empowered patient.

11 Next steps Continued stakeholder engagement.
Develop governance structure including all key stakeholders. Project team to develop the design to RIBA Stage 2. Development of Outline Business Case (OBC). Outline Business Case to be submitted to CCG and SEG for approval. On approval of the OBC Project team to develop planning application and progress the design to RIBA Stage 3. Tender for construction partner based upon single stage design and build using NW Construction Hub. Ongoing September ‘17 November ‘17 December ‘17 January ‘18 April ‘18 June ‘18
