A WORKING DEFINITION
Primary care networks enable the provision of proactive, accessible, coordinated and more integrated primary and community care improving outcomes for patients. They are likely to be formed around natural communities based on GP registered lists, often serving populations of around 30,000 to 50,000. Networks will be small enough to still provide the personal care valued by both patients and GPs, but large enough to have impact through deeper collaboration between practices and others in the local health (community and primary care) and social care system. They will provide a platform for providers of care being sustainable into the longer term.
Provides support for local communities, building on learning from the existing models;
Provides advice on the key areas commissioners and practices might consider in establishing primary care networks locally;
Sets out the vision for networks, core characteristics, care models at the heart of primary care at scale;
Identifies key enablers that underpin effective development of networks;
Shared widely for comments and we’d like your views – released in autumn.

3. Supporting the development of PCNs
We know that:
We are starting from a great position, with many already part of a network… but we have the opportunity to really support the journey to PCNs.
There will be multiple ways to facilitate development and we will have some choices to make.
In order for development of PCNs to be successful we will need to build momentum and ownership.
Co-production will be important if we want development to be successful and sustainable.
We have good experience to build from.
PCNs need to be locally driven.

Built from learning and experience about what works well in developing PCNs

Specific inputs and products on areas that need a single more technical response or enablement to enable PCNs to develop.
Products could include - ‘how to guides’ for example in approaching estate or – policy intervention to unblock barriers
National leadership development programme for primary care leaders
Focussed on primary care leaders for today and tomorrow

6. This could include:
Support for development of network leaders which specifically seeks to build leadership from a broad constituency including GPs (partnered and salaried/sessional), senior nurses and AHPs, pharmacists, and managers (practice, CCG and community services).
External coaching, facilitation, change management, contracting and funding models, service redesign and improvement support delivered direct to networks and through peer network activities across an STP/ICS and/or region.
Local network development e.g. diagnostic and transformation plan development sessions, rapid improvement cycle implementation.
Engagement and peer learning events for networks across STPs/ICSs and regions.
Subject matter expertise e.g. for network-level workforce analysis and redesign, population health, personalised care and patient engagement.
Technical support e.g. on estates, digital, workforce development etc.
