2 It all starts with a ‘digital transformation strategy’ – but what does that mean?
NHS England is using technology to empower patients and make it easier for clinicians to deliver high quality care and enabling patients to seamlessly navigate the service as part of its digital transformation strategy. This is being led at National but more locally, at South West London level to achieve economies of scale and in order to service multiple hospitals across borders and boroughs. How we are taking this forward? Self care & healthy living apps Interoperability Shared care planning across teams E-consultation IT Efficiencies – More time Online appointments/prescriptions - Better access Patients in control

3 1. Examples of how we embracing technology in Merton to benefit patients
The Online Consultation programme is a contribution towards this ambition. As part of the General Practice Forward View, a £45 million fund has been created to contribute towards the costs for practices to purchase online consultation systems, improving access and making best use of clinicians’ time. Funding is part of the General Practice Development Programme building capacity for improvement through free training, spreading innovations from around to country using the 10 High Impact Actions and funding for new ways of working. Benefits of online consultations Recent years have seen rapid development of a number of online consultation systems for patients to connect with their general practice. Using a mobile app or online portal, patients can tell the practice about their query or problem, and receive a reply, prescription, call back or other kind of appointment. They can also access information about symptoms and treatment, supporting greater use of self care. In early adopter practices, these systems are proving to be popular with patients of all ages. Many enable the patient to access information about symptoms, conditions and treatments, and connect to self help options. They free up time for GPs, allowing them to spend more time managing complex needs. As well as improving the service for patients, evidence to date indicates that online consultation systems can free up time for GPs to spend more time leading complex care for those who need it. Current status in Merton Creation of Task and Finish group and produce long list and short list of preferred providers (e.g. EMIS, Babylon, sensely). This group also has patient representation. South West London Digital Enabled Transformation Programme Objectives Our focus is to transform health and care for the residents of south west London through the use of digital technology to: Empower people to maintain their health and manage their illness Support clinicians to deliver high quality care at all times as part of a network of professionals, with access to the patient’s record and care plan Integrate services across health and care so that patients receive support and care in the place that is most convenient for them SWL Interoperability Programme Phase 1 One of the underpinning infrastructures necessary to enable delivery of our objectives is ‘system interoperability’. This is the linking of data and processes across health and care settings in SWL.  The SWL Interoperability Programme – Phase 1 aims to achieve this. An ETTF bid of £2.9m to fund delivery of phase 1, over a two year period, was successful and approved in September 2017. Phase 1 will deliver Access to view GP records in urgent and emergency care settings (111/OOH, A&E, Trusts, Community) GP access to view key information datasets shared by secondary care settings A single common Information Sharing agreement across SWL Interoperability - Phase 2 A business case to secure further ETTF funding by March 2018 to deliver phase 2 of the SWL Interoperability Programme, is being developed. Phase 2 will aim to deliver: SWL Integrated Digital Care Record that meets the needs of all 6 CCGs Social care integration with health Digital enabling transformation projects that align with the wider SW London digital strategy
