Congratulations! Due to your academic achievements, you are eligible to become a member of the Somerville Chapter of National Honor Society Please remember: Membership is highly selective Submission of completed forms DOES NOT guarantee membership Meeting of leadership & service criteria DOES NOT guarantee membership

Leadership Please review the specific details on what constitutes leadership in the Application Procedures You must be able to document at least ONE example of leadership on your Application Form for a total of 20 POINTS At least 6 months of service = 10 points. 2 or more years of service = 20 points. Failure to document 20 points results in ineligibility for the remainder of the application process This may include: Officer positions in school clubs, captains of sports teams, leadership positions outside of school (youth group leader, etc.). Your leadership role must be defined Students must detail and describe leadership and have it verified by the advisor/supervisor Ex) Secretary of Key Club: must have form signed by Ms. Lockwood Ex) CCD or Sunday school leader: must have form signed by church leader

Scoring for Leadership John has 1-year president of class council. John receives 10 points. John also has 6 months as vice president of his 4-H club. John receives 10 points. John also has been responsible for overseeing a youth group mentoring program for the past 3 years. John will receive 30 points. John’s total points for leadership will be 50.

Service Please review the specific details on what constitutes service in the Application Procedures You must be able to document 25 HOURS/POINTS of leadership on your Application Form Failure to document 25 HOURS/POINTS results in ineligibility for the remainder of the application process Your service hours should include activity both inside and outside of school: By Kids for Kids, scouts, 4-H, church group, volunteer organization (variety is key!) Students must detail and describe service activities and have it verified by the advisor/supervisor (this CANNOT be your parent)

Scoring for Service John has volunteered as a youth baseball referee for the past 2 years on Sundays for one-hour practices. John accumulated 104 hours. John has volunteered at his church’s youth group for the past 8 months. John accumulated 32 hours. John has accumulated the necessary minimum 25 points. NOTE: You are encouraged to show continual community service rather than one isolated incident. Example: What to include: John has been the neighborhood yard service (raking and shoveling) for the senior citizens for the past 2 years. John has accumulated 10 hours. What not to include: John raked his neighbor’s yard last fall. John accumulated 1 hour.

Application Form Access and complete the APPLICATION FORM on the NHS GoogleClassroom Please type your responses and review them Scan your Application Form to PDF with all signatures by Friday, March 9th, no later than 3pm to GoogleClassroom Please be thorough in your explanations. When listing your activities, please briefly describe your duties, activities, etc. DO NOT FORGET TO HAVE ALL ADVISORS SIGN OFF ON YOUR APPLICATION FORM

Personal Statement Access and complete the PERSONAL STATEMENT within the APPLICATION FORM You will answer the following questions: Question #1:  What attribute do you most admire about your character and what attribute do you least admire about your character?  Provide at least two anecdotes about yourself that best illustrate your character considering the questions above.   Question #2: Student leaders and the organizations they lead at school provide a large variety of services to the school and the community. Through these activities, they offer service that dramatically influences the lives of others.  Please give one example of how you have influenced the lives of others in a positive way. Explain how you demonstrate the ideal qualities of character, leadership, and service for NHS Please do not leave this blank! Type your responses and limit them to 250 words

Letter of Recommendation There is a form to provide the adult Choose ONE adult (outside of SHS) to write a letter of recommendation form about your leadership skills and commitment to service There is a form to provide the adult located on the APPLICATION PROCEDURES Letters of recommendation must be mailed or emailed to Mrs. Greenfield by Friday, March 9th IT IS YOUR RESPONSIBILITY TO FOLLOW UP WITH THE ADULT TO MAKE SURE IT IS ON TIME. FAILURE TO HAVE YOUR LETTER SUBMITTED ON TIME WILL RESULT IN A VOID APPLICATION!

Application Form Samples Sunday school teacher, 9th and 10th grades, organized and led class for 6-year olds on Sundays, 20 points Recording Secretary of Environmental Club, 10th grade, maintained club communication and increased attendance, 10 points Girl Scouts, Silver Award, 9th and 10th grades, organized food and supply drive for local animal shelter, 20 points Leadership: 11th Grade Vice President of Key Club, 11th grade, organized Thanksgiving food drive and helped raise $1,000, 10 points

Application Form Samples Service: 9th Grade Boy Scouts, participated in park clean up and senior citizen game night, 5 hours/points Volunteered for Habitat for Humanity building temporary housing in Point Pleasant, 10 hours/points Recycling pick up for Environmental Club at SHS, 3 hours/points Service: 10th Grade Volunteered at soup kitchen on Sundays organized by local church, 10 hours/points Tutored 3rd grade neighbor in math throughout the school year, 15 hours/points Service: 11th Grade Volunteers at Mathias School to spend time with various handicapped members on Wednesdays, 5 hours/points Volunteers at out-patient unit at Somerset Medical Center by providing refreshments to patients on Saturdays, 3 hours/points NOTE: If you are currently involved in a project or program, you can include it but it may not be considered by the committee.
