Consideration for Membership Page 3- Student Activity Information Form Required Personal statement It is vital that candidates devote their attention to the personal statement. Type an essay (1 page max), which highlights important experiences and achievements in your high school years. Make sure you include anything not covered elsewhere in the membership form. As you write, keep in mind the four cornerstones of the National Honor Society: Character Leadership Scholarship Community Service

Consideration for Membership Page 4- Consideration for Membership Remember that you must read and sign a statement pledging that you do indeed wish to be considered for membership. If for some reason you would not wish to be considered, it is expected that you will turn in the packet with this page completed ASAP! As indicated by the page in your application packet, your parent/guardian must sign this portion as well. This will ensure that both the applicant & the applicants parent/guardian have read and understood all of the directions and procedures involved in the selection process.

Consideration for Membership The application is due NO LATER THAN: November 5, 2019 Any form submitted after the deadline will not be considered for membership. You may only submit your application to Mrs. Forguites (Room 706) or Mrs. Lembke (Room 505). Any and all questions must be directed to Mrs. Forguites (emily.forguites@cravenk12.org) or Mrs. Lembke (erin.lembke@cravenk12.org). Teacher Recommendations will be sent out once the form is received by Mrs. Forguites & Mrs. Lembke. Teacher recommendation forms will be sent digitally to your selection teachers.
