Health and Wellbeing visits – what we do Arrange for older people to be visited in their own homes for a conversation with the volunteers about their health and wellbeing needs and priorities Offer basic information and advice together with the offer of a referral to an appropriate source of help Volunteers do a brief fire safety check and give advice about how to stay safe at home Evaluate each visit and offer a follow up visit to check the progress of any referrals Trained volunteers Over 50 with long term condition, disability or is socially isolated 17/01/2019 12
