2. Part 9 of the Social Services and Wellbeing (Wales) Act
Requires local authorities to cooperate with relevant partners including the NHS in the provision of care and support
Provides for partnership arrangements and pooling of funds between local authorities and LHBs for the discharge of their functions
Requires the establishment of partnership arrangements on LHB footprints, with oversight from Regional Partnership Boards comprising senior representatives from all sectors and users and carers
‘My care is planned by me with people working together to understand me, my family and carer (s), giving me control, and bringing together services to achieve the outcomes important to me’

3. West Wales Regional Partnership Board
One of 7 Boards established in Wales under Part 9 of the Social Services and Wellbeing (Wales) Act
Cross-sector membership as specified in statutory guidance
Principal responsibilities:
Bring together all relevant partners and ensure that they work together effectively to improve outcomes for people
Promote the integration of key services e.g. for older people, learning disability, carers and children with complex needs
Establish partnership arrangements and supporting pooled funding arrangements where appropriate
Ensure appropriate arrangements are in place to meet core statutory duties such as provision of Information, Advice and Assistance and advocacy

5. West Wales Population Assessment (2017)
Developed by working groups across agencies and sectors
Informed by public consultation undertaken in partnership with Public Service Boards
Identifies needs across different population groups and gaps and areas for improvement in respect of each

6. West Wales Population Assessment (2017)
Key findings:
Need for care and support is projected to grow in West Wales over the coming two decades, in terms of volume and complexity
Parallel reductions are expected in the number of working age people in the Region, raising challenges in terms of a sustainable workforce
Considerable foundations on which to build in terms of (1) existing good practice and (2) assets within the community that can be mobilised to support the preventative agenda
Significant numbers of people have caring responsibilities and consider having appropriate information and advice on support available for them and the people they care for within their communities

Developed by cross-agency working group and endorsed by the RPB and each of the statutory partners
Published in March 2018
Spans the next 5 years
Directly informed by the findings of the Population Assessment
Sets out high level strategic objectives which will be supported by a range of actions at regional and local level
Recognises the Five Ways of Working set out in the Well-being of Future Generations Act
Commits to a co-productive approach to implementation, including PSBs

9. A three stage preventative care and support pathway
Stay well and independent within the community – Prevention Stage 1
Maintain independence through provision of targeted support that prevents the need for people to be admitted to hospital or long-term residential care, or supports timely discharge – Prevention Stage 2
Provision of appropriate, outcomes-focused long-term care and support
– Prevention Stage 3

10. Staying well and independent in the community
Develop a shared Health and Wellbeing Strategy for the region which embeds prevention and reduces health inequalities through mainstream action across the whole system
Establish regional preventions framework based on effective local practice and aimed at building community resilience
Embed and promote Dewis and Infoengine as primary service portals, linked to the NHS 111 service and local Family Information Services
Establish and implement regional strategy for Technology Enabled Care (TEC)
Implement ‘The Right Help at the Right Time for Children, Young People and their Families’ regional threshold framework which includes support to reduce Adverse Childhood Experiences (ACEs)

11. Maintaining independence through provision of targeted support
Develop integrated community care model through locality- based community hubs, providing proactive low level care, step- up facilities and joined up management of chronic conditions, reducing admissions to hospital or long term care and supported by pooled fund arrangements
Review arrangements for Integrated Community Equipment Stores and implement regional model, including consideration of pooled fund arrangements
Ensure adults and children with autism and a learning disability or mental health problem receive appropriate, coordinated, targeted care and support through the development and implementation of a regional Integrated Autism Service
Ensure targeted care and support services meet the needs of people with dementia through delivery of the Regional Dementia Strategy

12. Provision of appropriate, outcomes-focused long-term care and support
Ensure IAA arrangements support people to make informed choices about their long-term care
Embed the Regional Adoption Service
Establish regional complex needs service, utilising regional resources where appropriate
Adopt single regional contract and service specification for older people’s care homes, supported by relevant policies and consistent approach to quality assurance and escalating concerns

Provides on line version of Area Plan and regularly updated data repository
