3 Cambridgeshire Population
Approximately 114,000 adults aged in 2011 99,000 adults aged 65 or over (forecast to increase to 140,000 by 2021) Approximately 7,000 Adult Social Care clients in ‘Older People’ category in March 2011 There will be corresponding increases, of over 50%, in people with dementia

But some people may need help to make best use of it

5 Care Network Cambridgeshire
An independent Cambridgeshire charity since 1986 Mission: To help older, isolated and vulnerable people living in Cambridgeshire to stay independent and maintain social contact with friends and the community. Vision: For a Cambridgeshire where all older and vulnerable people at risk of isolation, loneliness or ill health feel supported in their local community achieve greater independence & a healthier, happier life are enabled to support each other for mutual benefit.

6 Help at Home, Information @ GPs and Community Navigators
Care Help at Home, GPs and Community Navigators 142 Volunteers support people directly and navigate them to services and activities – recruited, trained and supported by CNC Network Community Development help to set up and support community groups Village Help Schemes Mobile Wardens Social Car Schemes - (over 500 drivers) Luncheon Clubs Practical Help Schemes

7 Local older people say they want
A range of social and community activities to combat loneliness Accurate, relevant, timely information “That bit of help” at the right time, from a listening ear to help with practical tasks A plan for a fulfilled older age Help for more isolated older people A key purpose of the Community Navigator approach is to respond to the views expressed locally by Cambridgeshire residents and stakeholders, through a number of consultation events including; Ageing Well, Somersham Community Planning and the Melbourn Locality Commissioning workshops. These consultations reinforce the findings of previous local and national consultations through highlighting that older people want to be able to remain in their own home and to live healthy active lives for as long as possible. A number of main themes have been identified as being important to older people including:

9 Projects – Community Development
Local offices and volunteers Supporting local people to identify and fill gaps Research community transport needs in the City Community Car Schemes - (over 500 drivers) – Other groups e.g. community cafes Mobile warden schemes Lunch Clubs Our Community Development team support and encourage local communities to set up and run groups such as lunch clubs, community car schemes, mobile warden schemes and a variety of other community group valued by older people. They also refer seamlessly to our Help at Home service who support individuals through a difficult time such as illness or following a hospital stay to regain their confidence and independence and to our new Community Navigator project which aims to enable older people to access activities and services which would maintain or improve their health, wellbeing and independence.

To maintain and improve the health and well being of older people in Cambridgeshire by: Informing people about and referring them to relevant activities and services Helping people overcome barriers to make use of activities and services Identifying where more activities and services are needed and working with local people to provide these Targeting people at risk of poor health and well being Reach out to isolated and lonely people at a local level, to reduce the (health and social) impact of loneliness, and to avoid the adult social care pathway starting with an expensive crisis, as far as is possible Raise awareness of, and connect people to, existing activities and services Ensure high quality information is available in key locations (e.g. with GP’s and Parish Councils) within communities, alongside trusted individuals who can help interpret it

Volunteers or funded staff might: Be trained to navigate residents, and others Coordinate local volunteers Housing schemes as venues for community activities.
