HAZARD HUNT since 2017 definition triggers Revitalised program Part of “back to basics” HSE operation Incidents Risk normalisation Coaching Improve RA Sync RL,BBS,FLS PDO Corporate Health, Safety and Environmental (HSE) team has launched a revitalized Hazard Hunt programme at all worksites in PDO. This is part of a “back to basics” HSE operation.   Through a walkabout, observations, hands on, HSE specialists accompany workers who are asked to participate and spot the hazards, identify risks, unsafe acts or conditions in their own workplace. Each of their findings will be photographed and then discussed with focus on the potential dangers and of course the best measures to prevent or mitigate them. In each case, the photo records will also shared with the relevant supervisors, HSE advisers and contract holders, ensuring that the risk assessments stay at front of mind as “live” documents. In some cases the team designs a Hazard Hunt poster for use with other colleagues. In recent years, we have seen an alarming trend of worksite hazards which were either disregarded or not identified to workers for them to be aware. What is also disturbing is that there were a number of repeat incidents which resulted in serious injuries.” HAZARD HUNT is not new to PDO. However, recent accidents is now driving a review of hazards and risk assessment. A PDO corporate team will be working to assist YOU in worksite hazard identification and risk assessment at YOUR worksite! Objective: to bring a Qualitative approach to hazard and risk assessment; in a practical way and to the work site level Reason: many recent PDO incidents involve personnel not knowing hazards or risk and unnecessarily putting themselves in the ‘line of fire’ at risk behavior! In 2017 Corporate HSE will be providing Hazard Hunt support and risk assessment coaching at the site locations. Hazard Hunt is all about bringing an improved HSE working environment to the front line staff those exposed every day The new Hazard Hunt methodology supports the new Behaviour Based Safety (BBS) activities, Front Line Supervision (FLS) and Reflective Learning (RL) and there will be a strong effort to synchronize and co-ordinate visits both to PDO and contractor sites.

Top 9 Hazards Challenges Goals Based on Historical Incidents’ Reviews and Analysis The Hazard hunt currently focuses on the PDO Top9 hazard exposures to tackle challenges. The exposures are related to our HSE data and specifically Lost Time Injuries. We fully expect to be able to reduce the accident frequency by concentrating on the work site activities and the hazards within. Our vision is to embed Hazard Hunt in all activities across the business and therefore to improve our HSE performance. Challenges Goals Involvement of stakeholders. Language barriers. Sustainability/quality of delivery Non productive time ( Production Vs. Safety) Fear factor ( hesitate to speak) Risk normalization Coaching and transferring knowledge to HSE advisors and front line supervisors/ workers (competence enhancement). Engagement with PDO and contractor site personnel. Improved risk assessments ( Keeping it live?) Synchronize the worksite hazard programs ( reflective learning, BBS, FLS) Overcoming risk normalization

But also the issue of how do we fight risk normalization, and how do we openly and honestly and productively deal with dilemmas When we started the activity we asked the team “if you want true engagement put your computer on sleep mode and your mobile phone on the drawer and just go and talk to people” and rly find out what's happening Tackling Risk Normalisation: Risk Normalization takes place when an activity becomes so familiar, or so routine, that you cease to see the potential areas of vulnerbility, and when you think about it its people becoming comfortable with circumstances that they shouldn’t be comfortable with, and it something we all can have occur in our lives. Nothing to be ashamed of but if your are walking/ working in a hydro-carbon plant where really dangerous materials float around and are piped around….. Then normalising risk can be quite a dangerous thing So we can tackle Risk normalisation by asking the workers open questions “What could go wrong? What are you worried about in the operation? Are you sure its going to be right every time you do this task? And having that aptitude about not telling people the answer, but asking the open question?” We don’t have a high degree of risk normalisation around baby's / toddlers We don’t just say: “Well, they haven't fallen off the satires yet, so that’s going to be OK.” we will always be alert to this. So how do we bring some of that way of thinking into our organisation? If something needs fixing, then lace up your shoes and do some organizing
