Importance Who has had an ache, pain, injury or issue during their military career? The medical exam done for all separating service members is used for many things. They help establish the TSM - Need for ongoing medical care as a result of medical issues. This is so that the TSM can be seen for their condition with the VA hospital post-transition and continual as the TSM needs - Can help the TSM make choices on their personal health care options. If they have specific medical needs or not it can help them to get the correct policy for their specific requirements - And help establish if there are any service connected medical conditions. This is beneficial for the TSM to get their VA disability rating either during the transition process or post-transition with the VA

Timeframes 120 days 60 days 180 90 30 days days days EAOS ~ The time frame for the medical portion is a simple process ~ Ending with your EAOS, or if needed the start of your Terminal Leave and possibly PTDY ~ 180 days: Schedule all necessary final dental and medical for family and self, including VA physical if applying for benefits pre-separation ~ 120 days: Have completed final physical either with MTF or, if filing VA benefits before separation, with the VA. We will cover the 2807-1 in a little bit, but this form needs to be completed before the Separation Questionnaire is endorsed. This is one document that is needed to send to PSD in order to get you Separation appointments scheduled. This can take 4-6 weeks depending on the amount of personnel transitioning and must be done in a timely manner. ~ 90 days: Family and self complete final dental and medical appointments ~ 60 days: Request copy of all medical and dental records. Make the copies of your paper records, yes records. Do not forget your dental record as well Don’t forget to also request copies of your ALTHA, or online electronic records. Plan ahead and make sure to ask for copies early. It can take up to 30 days depending on the amount of requests for records at your MTF. ~ 30 days: All documents complete and ready to start terminal leave or before EAOS. ~ By the time you get to your EAOS everything in the process should be done.

Identify and Document Report all injuries and illness Follow doctors orders Continue with treatment plans In order to have your medical conditions documents you must report them to your MTF and PCM Make sure that you follow the PCMs treatment plan and medical advise Follow through with the recommended plan of treatment

Ongoing Care Medical treatment and care Medication continuation Continued re-evaluation Ongoing care is the care that the TSM needs for medical issues post-transition and is done with the VA hospital. If the TSM is a dual military member this can also be accomplished with their PCM at a MTF and copies forwarded to the VA as needed. Medications are also included in this for condititions requiring them VA can require reevaluation for certain conditions to determine prognosis over a period of time.

Physical Examination DD Form 2807-1 DD Form 2807-1 (Report of Medical History Form) Multiple step/appointment form completed by the service member to address medical history. Locally the TSM will go into the medical clinic located onboard NBK Bangor and let the front desk staff know they need a Separation Physical Packet. They will complete the highlighted portion of the forms in their entirety. Be seen by Optometry, Audiology and Dental before returning to the front desk to schedule an appointment with their provider for the final physical portion. During their appointment the TSM will have their forms reviewed, notes made, and endorsed & finalized by the provider as medical ready to transition. This form packet is also endorsed & signed by both Medical and Dental to confirm that the TSM is both medically and dentally ready for transition from the service.
