Baseline Data Details Over ½ of discipline referrals for students with disabilities are due to disruptive behavior and anti-social conduct Climate survey results indicate that students dropping out are often associated with the feeling that they do not fit in, are disconnected with peers, and/or that their teachers do not like them.

Goal 1: FOCUS AREA: Student Engagement Implement Capturing Kids’ Hearts with Fidelity The Capturing Kids’ Hearts Program is about relationship building between our staff and students. Select staff were trained in the Summer of 2011 and additional staff are being trained in the Summer of 2012.

Goal 1 Data Targets By the end of the summer of 2012, at least 50% of the staff from all schools will be trained in the Capturing Kids’ Hearts Program During the 2012-2013 school year, staff will re-visit the core beliefs of the CKH Program at least quarterly. 100% of students will participate in at least one club. 100% of students will be able to identify an adult within the school with whom they feel they have developed a trusted connection. 100% of all administrators and staff will handle discipline issues in a fair and consistent manner

Goal 1 Strategies Training of Staff Include evidence of Capturing Kids’ Hearts in Administrative Walk-Thru’s Plan activities that foster relationship building between staff and students (CLUBS, REWARD DAYS) Implement a system of re-teaching during ISS with guidance and graduation coach Teach and revisit the school-wide discipline program as needed

Goal 1 Implementation Steps Staff will receive training on the CKH Model Staff will present to other staff (Teachers Teaching Teachers) the basic components of the CKH Model Students will build social contracts with staff that guide the climate within the school and classroom Staff and students will share in the planning of activities that foster growth in positive relationships within the school by collaborating on club development and activities/rewards

Goal 2: FOCUS AREA: Academic Engagement Develop a system of identification and support Students entering HHS who have been identified as being At-Risk will be provided additional supports to ensure success.

Goal 2 Data Targets 100% of staff will be trained in effective instructional strategies to improve student learning environment. The Graduation Coach will work with counselors to identify all students determined to be At Risk for non-graduation.

Goal 2 Strategies Students with Disabilities who have been determined to be At Risk will be paced in Learning Skills Classes to further guide their success. Teachers will train other teachers in the effective use of strategies in the classroom. The graduation coach will assist in the development of the identification of students deemed to be At Risk, the process for which a referral is generated, and the creation of support plans.

Goal 2 Implementation Steps Staff will be exposed to various teaching and instructional strategies throughout the school year during ISE Days and Faculty Meetings. The Graduation Coach and Counseling Team will develop a process of identification based upon attendance, behavior, and course failures. The Graduation Coach will develop a process for working with At Risk students, along with an Exit Survey to document reasons why students choose to drop-out.

Goal 3: FOCUS AREA: Behavior Be proactive…prevention and intervention are the keys to student success Students at HHS will feel that they are treated fairly, respected, and that they are a part of THEIR school.

Goal 3 Data Targets 100% of students will be trained in the discipline plan. 100% of all administrators and teachers will handle disciplinary issues in a fair and consistent manner. Graduation Coach will use the ISS/Detention Room for an opportunity to teach/re-teach behaviors 3 days a week. Staff will refer students with consistent behavior issues to guidance, the graduation coach, a student mentor, or to the Student Assistance Team 100% of the time.

Goal 3 Strategies Staff will model the CKH standards by developing with students a Social Contract within their classroom. Staff will work together in identifying students that they have a connection with and mentoring those students. The school discipline model will be taught extensively to staff and students and revisited in Advisory, Developmental Guidance, and ISS during the school year.

Goal 3 Implementation Steps Staff are trained in the Capturing Kids’ Hearts Model. Staff and administrators will be trained in the discipline model. The Graduation Coach, along with guidance counselors, will assist students with disciplinary issues in developing acceptable replacement behaviors to aid in improved behavior and acceptable social conduct.

Goal 1 Results By the end of the Summer of 2012, nearly 50% of the staff at each school will have been trained in the CKH Model. Social Contracts are being utilized within classrooms and during faculty meetings. Administrators take note of classroom climate during the conducted Walk-Thru’s Club offerings at Hampshire High School have increased from 14 in 2010-2011 to 23 in 2011-2012 with 27 staff members serving as club advisors. Student end of year survey results will determine whether students feel connected to an adult

Goal 2 Results Teachers teaching Teachers has become commonplace during faculty meetings and on ISE Days to support quality instructional practices in the classroom. Graduation Coach has developed a system for working with the At Risk students 2 days a week one on one. Graduation Coach has developed an exit survey to be used with exiting students during the 2012-2013 school year.

Goal 3 Results Time period August of 2011 through May of 2012 shows improvement in Discipline Offenses in comparison to same time period last year. Areas showing improvement include: Physical Fighting, Verbal Assault of School Employees and Students, Harassment/Bullying, Threat of Injury, Bomb Threats, Battery Offenses, Possession/Distribution/Use and Sale of Illegal Drugs

Graduation Rate Projection It is projected that HHS will have a Graduation Rate improvement this school year that reflects a 78%-80%

Dropout Rate Projection With 13 seniors exiting early to date, the projected Drop-out Rate is 0.05

Post School Outcomes Projection It is hoped that the post school outcomes of students with disabilities graduating from high school will see an increase in the amount of students transitioning to some level of continuing education. It is further hoped that the local businesses continue to offer career placements to our students, as they have in the past.
