Expeditionary Combat Readiness Center Medical Deployment/Redeployment //UNCLASSIFIED//

Medical Department ECRC Mission: The mission of ECRC is to provide processing, equipping, training, certification and family support to Navy Individual Augmentees, provisional units, and ADSW Sailors throughout all deployment phases in support of COCOM requirements, contingency operations, or national crises. Medical N9 Mission: To provide thorough deployment medical screenings for all Sailors processing through ECRC, and complete separation physicals and referrals for care as necessary. //UNCLASSIFIED//

Medical Processing Deploying Sailors are required to have the following items completed prior to reporting to ECRC for processing Required Medical Forms -DD 2807 – Report of Medical History -1300/4 – Expeditionary Medical and Dental Screening Ancillary Services -Dental Exams -Audiograms -Optometry -Lab work -Pre-deployment Health Assessment (EDHA) -Immunizations -Automated Neuropsychological Assessment Matrix (ANAM) -Periodic Health Assessment (PHA) -Women's health examinations

Deployment Verification Form (DVF) Upon initial check in, the medical team collects all records and uses the DVF to screen members medical readiness. The records are returned at the end of the day and all items not completed will be highlighted.

Medical Processing begins the second day of the week and includes; Vital Signs assessment and entry in AHLTA Thorough medical record screening Pre-deployment Health Assessment (EDHA) screening Ancillary services review and completion Provider interview (all members are required to see our providers) Malaria Medication distribution and brief Check out with Corpsman

Medical Processing Once all medical items are complete, the Corpsman will sign off the processing checklist (shown below) and members will report to the processing department as directed. Medical/Dental Screening (DAY 2) Completion of PDHA Labs (as applicable) Briefs with Corpsman Provider Interview Medical Assistant (MA) Screening / Immunizations / PPD (as applicable) ____________________________ _________________ Initial Medical Clearance Date Additional Medical Screening as Necessary (DAY 2-3) Audio/Dental/Optometry / Immunizations / PPD READ Malaria Medication Received Labs & Specialty Appointments: HIV, PSA, Lipids, Glucose, Paps/Mammograms and EKGs ____________________________ ___________________ Final Medical Clearance Date

Medical Demobilization Re-deploying Sailors are required to complete the following prior to departing ECRC Required Medical Forms -DD 2807 - Report of Medical History -DD 2808 - Report of Medical Examination -DD 2697 – Report of Medical Assessment -Post Deployment Health Assessment (online) Ancillary Services -Audiograms -Lab work

Medical Demobilization Members returning home from mobilization and separating from active duty service are required to complete a Separation Health Physical and Evaluation (SHPE). The SHPE takes approx. 45 minutes to complete and can be preformed at a VA facility, Military Treatment Facility or DoD contracted service. ECRC will facilitate the SHPE if not previously completed. Members filing a VA claim must complete the SHPE within 90 of their date of separation from active duty. The SHPE should be completed with a VA when possible.

Medical Demobilization Medical delay/Medical Evaluation Members returning from a mobilization may receive care for their illness or injury at ECRC at the providers discretion. The Case Management department manages the medical care of all members enrolled in the medical delay and evaluation programs. (Medical Delay-stays on current demobilization orders for up to 28 days. Medical Evaluation-orders requested from PERS for an additional 90 days) Members requiring care beyond 118 days will request orders for treatment at MEDHOLD (RCC Mid Atlantic)

Medical Demobilization Resource Referral Tracking Manager Members returning home with a medical condition are given the option to return home and receive care with a Home of Record (HOR) referral. The Resource Referral Tracking Manager (RRTM) researches VA and civilian facilities to meet the needs of the patient and manage the HOR referrals.
