Public Health Division Since 1987 Currently provide 3.0 FTE nursing time at the health unit 2.5 support time Mon-Fri: 8:00 a.m.- 9:30 p.m.

Stearns County and RAP 2002 Goals for the Correctional Health Unit One of 5 goals: Increase coordination-Human Services Programs prior to inmates release reduce recidivism crisis prevention 2003 implemented concept of RAP team

RAP Collaborative programming between Public Health lead Human Services Sheriff Office-Jail Administration Public Health lead Concept discharge planning

Purpose of Release Advance Plan Multi-discipline team-similar to hospital discharge planning Address post-incarceration needs Goal of impacting recidivism

2003 RAP-5 Inmates

RAP Summary-July 2003-Dec 2003 1st year of the team 5/7 inmates Incarcerations 2 years before/after RAP Impact: 20% decrease in the # of incarcerations

2004 RAP-26 Inmates

RAP Summary-2004 2nd year of the team Incarcerations 26/29 inmates 87 less incarcerations 2 years prior to RAP and 2 years after RAP Impact: 65% decline in # of incarcerations

2005 RAP-20 Inmates

RAP Summary-2005 3rd year of the team Incarcerations 20/23 inmates 81 less Incarcerations 2 years prior to RAP and 2 years after RAP Impact: 72% decrease in # of incarcerations

Eligibility Criteria Inmates identified by jail administration or public health nurses or by inmate Diagnosis of serious and persistent mental illness Significant social or health concerns/needs Identified CD/drug dependency issues Homeless upon release Willing to participate in RAP Stearns County resident/releasing to Stearns residence

RAP Process Initial meeting with inmate Short questionnaire Jail Administration or PHN in jail Short questionnaire Determines eligibility Preliminary direction

RAP Process If inmate interested in participation Consents-releases obtained Schedule on RAP agenda Staff notified of RAP participants Bring pertinent information to RAP PHN meets with Inmate to do more in-depth assessment.

RAP Activity Inmate participation Introductions/Sign-in Begin reviewing needs and problem-solving priority needs Facilitator notes activities to be completed Summary provided to all team members

Preliminary Information Beginning to track services provided For 167 inmates 37% had CD issues 24% had MI issues 13 % had both CD and MI issues 41% had Medical Issues 66% had Financial issues 49% had Employment issues 31% Housing issues 63% had at least three issues to address
