Introduction Correctional Facility: Scott County Scott County is located in the 7-County Metropolitan area of Minneapolis/St. Paul Population of Scott County is 137,250 (2013 census) Scott County

Introduction Correctional Facility: Scott County The Correctional Facility is made up of 6 pods, 4 male and 2 female Average population inmates Age range of these inmates is between years A large number of these inmates are characterized as “homeless” upon their admission Many of these come from the streets of Minneapolis/St. Paul by a free bus service provided by a local casino

Healthcare Issues Encountered These individuals frequently have issues related to possession of illegal drugs and chemical dependency Many have overlying mental health issues (70%) Illnesses related to Sexually Transmitted Diseases HIV Hepatis C Hep B Syphilis Gonorrhea Chlamydia

Previous Healthcare Services Provided 2 Full-Time Registered Nurses provide daily healthcare resources to the Correctional Facility Sunday, 6am-4pm Monday-Friday, 6am-4pm After-Hours “on call” coverage 1 Physician Medical Director 2 Clinics provided each month 4 hours per clinic 20-25 patient encounters per clinic On call back-up coverage for Nursing staff questions

23 Outcomes Projected A greater onsite presence of healthcare providers within the Correctional Facility should lead to a: Decreased need to transport ill or injured inmates for offsite care Decreased cost for Emergency Room/Urgent Care Decreased costs for extra security personnel needed for offsite management Decreased cost for overtime hours paid to Correctional Facility staff for offsite healthcare issues

24 Outcomes Projected Upon inmate discharge a greater onsite presence of healthcare providers should allow: Additional opportunities for Correctional Facility staff to develop a follow-up Plan of Healthcare Additional opportunities for Mental Health staff to develop a follow-up plan of Mental Health/Chemical dependency care

26 Conclusion The Community Paramedic, placed within a Correctional Facility, can contribute greatly to the level of services provided by the Healthcare team By adding this Extender, the Correctional Facility could achieve a higher level of in-house care, by increasing efficiencies and reducing overhead costs By adding this Extender, additional time could become available for the Healthcare team to develop a discharge plan of care (A continuum of care)
