1 CIVIL STATE MENTAL HEALTH TREATMENT FACILITY RESPONSIBILTIES

2 Purpose LSF Health Systems (LSFHS), in collaboration with its network providers is committed to ensuring coordination with State Mental Health Treatment Facilities to assist the DCF in its goal of reducing admissions and length of stay for adults in state hospitals and return them to a community environment.

3 Point to Remember: The goal is the successful reintegration of individuals discharging from a state hospital back into the community and to reduce, divert and work to prevent re-hospitalization of this priority population.

4 State Mental Health Treatment Facility Staff Role and Responsibilities Per CFOP 155-17, during the community discharge planning process, the designated state mental health treatment facility staff will: Coordinate the discharge planning meeting. This meeting includes: the resident, recovery team, guardian, community case manager/forensic specialist/forensic case manager, family, and others as appropriate. Assist the resident in obtaining financial resources necessary for funding the community placement prior to discharge (i.e. SSI, SSDI). Complete initial application for benefits and disability determination and verify that the persons returning to the community are eligible for reinstatement of pay status upon discharge. Assist the resident and case manager in locating appropriate community placements. This is accomplished by arranging site visits and preparing/submitting referral packets to the case manager and/or community placements. Coordinate the documentation and paperwork necessary for acceptance into community placement (i.e. obtaining copies of birth certificates, IDs, immigration paperwork, etc.).

5 State Mental Health Treatment Facility Staff Role and Responsibilities Coordinate the completion of the State Mental Health Facility Discharge Form (CF-MH 7001) and submit the form to the community case manager/forensic specialist/forensic case manager prior to discharge. Provide the community case manager/forensic specialist/forensic case manager with at least seven (7) days notice of the anticipated discharge date whenever possible, or three (3) days notice when a resident is court ordered or is voluntary and requests discharge. In collaboration with the case manager: assist in developing a social support system in the community, prepare resident for employment upon discharge if appropriate, help prepare resident to take as much responsibility as possible for addressing their medical and psychiatric needs upon discharge. If the resident is under criminal charge at the time of discharge, the administrator shall transfer the resident to the custody of the appropriate law enforcement officer upon discharge.

6 Send the calendar of treatment team meetings to the ME for distribution to providers. Notify ME if there are issues concerning a provider that need to be addressed. Reach out to ME for assistance with creating continuity of care between providers and NEFSH. Send ME liaison a weekly report of discharged consumers.
