Our journey Developing an MSK online support service (Web and App) Over next 12 months review clinical pathways to assess opportunities for further developments

Our journey Currently implementing FLO in the Heart Failure Service. Will continue its use across a range of clinical services. We aim to offer tele/video GP appointments. As Care Co-ordination matures, we aim to look at how we can operationalise and mainstream the use of both Telehealth and Telecare.

Examples of Local Innovations underway FLO Telehealth MSK on Line support and App Rally Round

FLO Telehealth A simple SMS based telehealth system in use across the NHS and across many clinical areas Able to attract into the CCG some investment to kick start this work. Is protocol driven, sending automated messages to patients which the patient responds to. These could include: BP Weight SPo2 Blood Glucose Levels etc System tracks and monitors patient signs and alerts the clinicians to unusual readings which can be tracked on any device. Easy to use, fast to deploy and cheap to run We are starting in Heart Failure first, moving rapidly onto other clinical areas.
