Self-managed care Virtual services will support the delivery of the hub and spoke model. SH:24 will provide: free STI test kits for women and men, including men who have sex with men sexual Health information and advice – 24-hours-a-day, seven-days-a-week through talk by text, call back and web chat with a nurse.

Clinic integration: - User-centred (we develop what users want and need, not what we think they need) / Collaborative / iterative (prototyping saves time, money and manages risk)

Psychosexual therapy services Interlinked with the universal hub and spoke model. Integrated with the psychosexual therapy services in Kent and overseen by the lead psychosexual therapist. Referrals, including self-referrals, will be triaged through the hub. Clinics will operate from 4 Clover Street, Chatham and identified spoke sites dependent on need.

Outreach and targeted HIV screening A comprehensive and collaborative outreach offer will be provided by METRO, HACO, Open Road and KCHFT. Specific remit to work with high risk groups, offering tailored intervention programmes to reduce the risk of poor sexual health and engagement in universal services. The outreach service will: work collaboratively to a Medway Sexual Health Outreach Plan take the service to communities through online platforms, events, one-to-one work and groups work in partnership with key stakeholders to build capacity, deliver sexual health promotion and innovative education interventions use data and feedback to understand changing populations.

HACO will work with black African communities in Medway and will: deliver targeted HIV POCT to black Africans in Medway deliver a peer support programme deliver a volunteer programme co-ordinate sexual health promotion through shops, barbers, hair salons, youth groups and black African events in Medway work with local faith leaders to promote positive messages around sexual health and to promote access to universal services. Open Road will provide a substance misuse practitioner to support the integration of substance misuse issues and sexual health for young people including targeted one-to-one work. Turning Point will support the sexual health agenda for adults with substance misuse issues by jointly delivering interventions aimed at vulnerable groups, for example sex workers, and running clinics in tandem with sexual health.

METRO will deliver: Sexual health promotion targeted at groups most at risk PitstopPlus online service for MSM Boys and young men’s programme Medway Wellbeing Month - a multi-agency month of health improvement activities in Medway, incorporating National HIV testing week and World Aids Day HIV point of care testing (POCT) to MSM and sex workers.

Condom distribution scheme Metro will co-ordinate delivery of Get it condom scheme. Get it will integrate will the full spectrum of young people’s sexual health services. Integrates local registration and distribution sites throughout services - includes GPs, pharmacies, young people’s services, looked after children, youth offending teams etc. Distribution points will be supported through an accessible website, which enables self-registration and ordering facilities for young people aged 16 to 24. Those aged under 16 will register at a physical location. METRO will offer training to key stakeholders to provide chlamydia screening in addition to the Get it programme. Maximises the use of volunteers and community champions.

PitstopPlus for high risk MSM

Chlamydia screening programme A centralised Chlamydia Screening Team delivered by KCHFT will: co-ordinate the delivery of the Chlamydia Screening Programme across Medway supply all advertising, support materials and testing kits for partners manage all results which are done through Bioscience focus on timely access to results and treatment, support partner notification and help re-testing. Home chlamydia testing will be integrated into pathways for online STI testing. The outreach team will carry out targeted chlamydia screening in high-prevalence groups.

Pharmacy sexual health services Co-managed with KCPP and other pharmacy chains, including Boots, Asda and Lloyds Pharmacy. Invigorates delivery of sexual healthcare by pharmacists through a mandatory offer of: Chlamydia screening and treatment, emergency oral contraception (Levonelle and EllaOne) and condom distribution. Ensures all who are eligible have access to services within 30-minute journey, accessible Monday to Friday 8.30am to 8pm and at least two hours at weekends.

Pharmacy services across Medway

LARC fitting and removal in primary care SLAs will be established with primary care with auditable outcomes, overseen by our training programme director. Training will be carried out within the hub as well as in primary care settings. Continuing support will be offered to share best practice, including opportunities for audit and clinical supervision to support future professional accreditation and re-validation.

Out of area budget KCHFT will manage the budget for out of area payments. Providers will have a strong focus on reducing the number of patients accessing services outside of Medway while recognising patient choice.

Our number one priority is making sure we involve young people in how we design our services. When we designed our service model for Medway we talked to lots of young people about where and how they would like to use sexual health services and listened to what they were telling us. We also know that the needs of young people change over time so its important to us keep talking to young people so we can continue to make sure we listen to ideas about how we can change our services for the better.
