2 Islington CCG Who we are and what we do Primary care practices
We are made up of 34 GP practices working in partnership with local providers and Islington Council Local health services We plan, buy and monitor hospital care, community health services, mental health and rehabilitation Integrated care Islington is an integrated care pioneer, meaning that the CCG and Islington Council work together to link services in health and social care

3 Joint working with Islington Council
Our four shared priorities are: To prevent and manage long-term conditions to extend the length and quality of life, and reduce health inequalities To provide high-quality, efficient services within the resources available To make sure every child has the best start in life To improve mental health and wellbeing To help us provide the best service, we listen carefully to what local people tell us. We meet regularly with them so they can ask questions and tell us what concerns them about their care. The services we buy reflect their needs.

5 Our plans for 2015/16 and beyond
Developing Islington’s health and social care workforce through our Community Education Provider Network Building clinical leadership with insight from Islington’s GPs to drive commissioning in partnership with patients and service users Working across north central London to jointly commission general practice to improve the quality of services and care for local people

6 Children’s Health Commissioning Team
Responsible for commissioning Health and Care services for CYP across Islington including service development Delivery of our 5 year ‘Children’s Health Strategy’ Underpinned by engagement and co production of CYP and Parents / Carers

7 Our Vision Our vision is to improve the health and wellbeing of children and young people in Islington from conception to adulthood and to reduce health inequalities by: Promoting good health. Making safe, high quality, affordable and coordinated health services available at, or close to home in partnership with children, young people, their parents and carers. Supporting them to be in control of their own health where possible and to maximise their life chances as they grow up.

8 Principles Prevention, early identification and intervention across all children and young people’s health services, from conception to adulthood, and other services which impact on children and young people’s lives. Equal access for all to personalised, high quality services, where and when needed, free at the point of access and with choice where possible/appropriate. Working in partnership with children, young people, parents, carers and their communities to be involved in the design of health services that promote good health and empower them to better manage their own health and wellbeing. Services within, and outside of Health, working together to deliver care coordinated around and responsive to the child, young person and family.

9 Principles Ensuring that safeguarding underpins all planning and delivery of health services to children and young people with the full commitment of all professionals. Making the best use of resources in commissioning services based on population need and the best available evidence

11 16/17 achievements… Re-procurement of young carers service
Procurement of ASD review which had strong input from Parent /Carers and service users via Ambitious about Autism. To consider the Assessment and Diagnostic Pathways and in particular current waiting times Hospital at Home Model ongoing

12 16/17 achievements…… Development of Mental Health Charter
Integrated health offer into the YOS Crisis care plans for CAMHS services linked across with Acute Trusts. Asthma in Schools Work

13 Working across different localities
Across Islington Across Haringey and Islington Across North Central London Population based approaches where it makes sense to commission across different population areas

14 Haringey & Islington Wellbeing Partnership
A range of organisations are working together: Haringey Council, Islington Council Whittington Health, Islington Clinical Commissioning Group, Haringey Clinical Commissioning Group Camden & Islington NHS Foundation Trust, Barnet, Enfield & Haringey Mental Health NHS Trust Haringey GP Federation, Islington GP Federation UCLH and NMH closely involved

15 So what will be different?
We see ourselves as sharing collective responsibility for meeting the health (mental and physical) and care needs of our population in the long term Focus on preventing illness through a ‘population’ approach Bring together resources of all the partners to improve health and care Extend multi-professional teams (long term conditions, mental health) based around GP practices, and plugged into local networks and voluntary sector Identify people putting their health at risk and prevent deterioration into statutory services Take cost out by increasing efficiency (reducing admin and duplication) and improving outcomes
