What’s happened so far? December 2015 – STP guidance launched June 2016 – outline of NCL-wide plan produced, workstreams defined October 2016 – first full draft produced for consultation, governance in place, more focus on public engagement March 2017 – plan updated, with more detail on ‘what, how and by when’ April 2017 – NHS Five Year Forward View updated, sustained focus on STPs and accountable care systems

Our Wellbeing Partnership: working together with people in Haringey and Islington

Who do we mean by the Wellbeing Partnership? A local footprint within North London, with a range of organisations working together: Haringey Council, Islington Council Islington Clinical Commissioning Group, Haringey Clinical Commissioning Group, Whittington Health Haringey GP Federation, Islington GP Federation Camden & Islington NHS Foundation Trust, Barnet, Enfield & Haringey Mental Health NHS Trust UCLH Foundation Trust and North Middlesex Hospital Trust

So what will be different? g So what will be different? We see ourselves as sharing collective responsibility for meeting the health (mental and physical) and care needs of our population in the long term Focus on preventing illness through a ‘population’ approach Bring together resources of all the partners to improve health and care Identify people putting their health at risk and prevent deterioration into statutory services Extend multi-professional teams (long term conditions, mental health) through CHINs initiative, based around GP practices and plugged into local community networks and voluntary sector Take cost out by increasing efficiency (reducing admin and duplication) and improving outcomes

Developing the Partnership Agreement Planning: working to a joint Health and Wellbeing Strategy Designing: bringing together our service redesign work Scoping services: agreeing which services are included Finance: taking decisions about spend & savings collectively Delivering: forming a joint management structure Monitoring: reporting together rather than separately Decision making: designing a system of decision-making that enables joint working Quick recap Organisations who are involved Following Vanguard bid and Value Based Commissioning work together on diabetes and mental health, agreed it would be useful to continue work and ‘formalise’ partnership Adopted an open approach to the partnership – not excluding organisations Focused on improving health and care for residents of Haringey and Islington Does not preclude working with other local boroughs where helpful

Wellbeing Partnership Board Sets strategic direction across member organisations and agrees local delivery of STP interventions and joint efficiency plans Holds accountability for delivery of outcomes Joint oversight of key financial decisions (funding, spend and savings) Programme Delivery Board (Clinical, professional & operational) Holds responsibility for implementation of the joint work programme Reviews and monitors progress across all areas and reports back to Wellbeing and other organisational Boards Ensures clinical / professional / operations leadership in place (including housing, voluntary sector etc.) Community Reference Committee Oversee community engagement & development, self care and communications throughout partnership and all workstreams Assure and measure community engagement & development, self care, communications & equality & diversity throughout workstreams, CHINS and partnership. Prevention & Wider Determinants of Health Population wide systematic implementation of prevention and maintaining independence initiatives – Support all programme areas to include focus on prevention, independence and wider determinants of health (children and adults) Care Closer to Home (1 Care) Wellbeing: CHINs, LTC management (diabetes & CVD) Urgent & Emergency Care Wellbeing: Frailty, intermediate care Mental Health Wellbeing: Prevention and Community Resilience; community MH Learning Disabilities Wellbeing: Improving health, wellbeing & opportunities and consolidating costs Children & Young People Wellbeing: Transition, A&E attendances, LTCs (respiratory) Elective Care Wellbeing: MSK incl. community, (gastro. Is an area of need for both H&I?) Information and Analytics Needs assessment, population and service information and analytics, outcome monitoring, integrated digital care record and e-communication Finance and Performance Group Technical development & monitoring of shared savings & performance plans – Support all programme areas to develop finance, activity and impact models
