2 What’s going on: Greater Manchester

3 Devolution: What is it? Greater Manchester now has control over £6 billion The focus will be on – People taking charge of their own health and wellbeing More care for people in the community and leave hospitals for specialist care Hospitals working together to make sure knowledge is shared and everyone has the same high level of care Changes which will make sure everything is fair as well as saving money We can’t do this alone – we all have to work together so we can start well, live well and age well! These powers have now been handed over as of 1st April 2016. For example, giving people more access and control over their health records.

5 We need to make some changes!
That’s Greater Manchester – what’s the plan for Bolton? If we do nothing and carry on as we are – by 2020 there will be a gap of £162million! By working together we can save £84 million – but we are £78 million short! So we can’t think small – we all need to make big changes We need to make some changes! Everyone in Bolton needs to get behind this We need to do more for ourselves, including staying healthy, managing a long term illness, or using services properly Hand out - copies of locality plan summary This plan is all about our vision for Bolton and it supports the devolution in Greater Manchester.

Urgent and Emergency Care Planned Care Mental Health £28.8m Learning Disabilities Integrated and Community Care Strategy and Planning Groups for each of these – report into the System Sustainability and Transformation Board who are held accountable for performance and delivery by Health and Wellbeing Board Care Homes and Home Care Children’s and Maternity

Transforming work programmes underpinned by 5 enabling programmes: Workforce IT and Technology Enabled Care Co-design Estates Matrix working between enablers and also with transforming work programmes LCO Development

12 Exciting times but a lot of work and a long road ahead…
Review Locality Plan and Transformation Fund bid to make sure it’s current and fit for purpose Formally sign investment agreement and agree terms Develop work programmes in more detail, including action plans Review governance to make sure it’s representative, inclusive and efficient

14 All of us in Bolton – commissioners and providers – need to change the way we plan and deliver services Working together more closely across the CCG and Council to commission seamless integrated care Integrating commissioning across the CCG and Council Economies of scale – what should we commission at GM, sector and locality level? New contract models to share risk and incentivise reform Outcomes based commissioning Commissioning for social value Focus on common standards and expectations to maintain and improve quality Improved focus on co-design – with staff, providers and the public

We expect the Locality Plan and new ways of working to create exciting opportunities for our VCS partners Focus on wider wellbeing, including social, economic and environment Considering wider determinants of health – employment, social, education, housing Commissioning for social value Greater VCS involvement in service planning and delivery Greater appetite for risk to trial new services and ways of working Creating space for new providers to create a diverse and vibrant market What’s your unique offer?
