DEFINITIONS  Safety is control of accidental loss (personnel, equipment, production, or environment).  Accident is an event which results in a loss (personnel, equipment, production, or environment).

1.Safety as a matter of fact  Safety first  Safety is a common sense  Safety is an alertness  Safety is an ability of prediction  Safety is knowledge and experience.  Safety is everyone’s business  Safety is a management commitment and responsibility

2.Safety in Design:  General layout, safe distances, exit / egress, wind direction….etc  Hazard operability study (HAZOP)  Fail danger / fail safe  What if??  Risk identification and control.  Trip systems logic and consequences.  Safe guarding systems.  Trip systems reliability.  Reliable, effective and testable alarm systems.  Gas, fire and smoke detection and alarm systems.  Consider maintenance related aspects.

3.Safety in Construction:  Heavy lifting equipment reliability.  Protective cloths and equipment.  Co. ordination  Housekeeping

4.Safety in precommisioning / commissioning and start up  Check up of lines, tubes and gauges.  Internal cleaning.  Hydrostatic & pressure testing.  Connections, spading / despading.  Running equipment  Safe guarding and alarm systems reliability testing.

 Management modifications (plant changes):  It should create no or acceptable risk.  It should be safety integrated into the whole plant system.  Safe guarded and protected.  It should realize the area classification.  It should cause no restriction or lay-out /accessibility problems.  Condition monitoring.  Effective maintenance programme  Human factors  Contractors safety

 Risk assessment / management:  Identification of the risk.  Event frequency.  Assessment of the consequences.  Escalation analysis.  Risk calculations.  Identification of risk reduction measures.  Means to contain the unavoidable risk and eliminate its consequences.  Safety enhancement techniques (targets and objectives, motivation, effective communication, rewarding system……..).
