Case Conferences Minimum expected attendance: Prison Based Social Worker Community Based Social Worker Case Co-ordinator Prisoner (though it continues in their absence if they decline) Clear focus on Risk & Needs Assessment Action Plan agreed with prisoner Sequencing of interventions discussed Allows for a smoother transition from custody to community Minutes confirmed with attendees, and then attached to CIP and sent to Prisoner and Community Based staff Relevant referrals made and work undertaken
