 The trip will take place from: Thursday, June 23rd through Saturday, June 25th.  This is the weekend immediately following the end of the school year.

 Our next, and last MANDATORY meeting prior to going on the trip will be:  WHEN: THURSDAY, June pm  WHERE: Meeting will take place in the I.H.S. Large Cafeteria  WHAT WILL BE COVERED:  Safety while in DC;  Finalized Itinerary:  Departure & Return;  Packing Questions;  Meeting the Chaperones;  Individual Group Meetings with Students/Parents/Chaperones

As a District Trip, we need to make sure that each student: continues to demonstrate that they are following all academic guidelines as noted in the District’s participation policy follows the Code of Conduct – is the behavior of the student appropriate enough in school to earn the privilege of going 386 miles away? attends school regularly and responsibly has paid the full balance for the trip

 On Thursday, June 23 at 4:15 PM, four Dake Jr. High students will have the privilege of laying a wreath at the Tomb of the Unknown Soldier in Arlington National Cemetery on our Washington, DC trip.  To determine who will take part in this once-in-a-lifetime experience, simply write a short essay on the topic below.  The authors of the four most thoughtful essays will be our 2016 wreath layers.  Deadline to submit is Friday, April 29,  Here’s the question…  Name a famous American you admire. Tell us why you chose this person and how he/she has impacted our country.

 In order for our school to be able to participate in yet another ONCE-IN-A-LIFETIME event, we must collect Social Security Numbers from students and chaperones for background checks.  Please complete the information requested on the slip provided. Seal the envelope. Once we record the information, it will be sent to the travel agent and we will shred all paper copies.

 All medical forms are due to Ms. Brayer in Dake Room 133 by Friday, April 15,  Each form will need to be checked and approved by our School Nurse, Mrs. Bottiglier.  Mrs. Bottiglier will need the proper amount of time to assess each form and provide copies to chaperones for emergency purposes.  No forms can be accepted the morning of the trip

Medications on the Washington D.C. Trip  A written prescription/documentation sheet is required from your child’s doctor for ANY/ALL medication that will be taken on this trip.  Your child’s chaperone will carry all medicine that may be needed during the trip, including over-the-counter medications UNLESS we have a doctor’s note allowing your child to self-carry.

Medications on the Washington D.C. Trip  On your medical form, please indicate to us any medication your child may need to take. This includes routine prescription medication(s), over-the-counter medications, and those used for emergency situations.  Any other medical questions that you may have related to your child may be directed to Mr. Reininger, Ms. Brayer, and/or Nurse Bottiglier.  **If you or your doctor would like to fax the forms into the school in a confidential manner, please send it to Nurse Bottiglier’s Office Fax Number (585)

 All medications that will be taken on trip need to be brought by the parent/guardian to Mrs. Bottiglier by Thurs, June 16. The Nurse’s Office hours are from 8:00 AM until 3:00 PM on school days.  Is you child on a gluten-free diet? Please let us know tonight!

 4 roommates to a room!!  Students may not privately switch rooms on the trip.  Please put the index card with your name and your chosen roommates names on the piece of paper.  Please do not touch other people’s cards or move any names around.  Roommates will have the same chaperone and be on the same bus together.  Remember, you will be with your roommates for 8 hours a night and with your bus members for 16 hours each day!  If only one parent/guardian is present tonight, please call the absent parent/guardian on your cell phone to ensure that all selections are verified.  Please hand in your roommate request paper to Mr. Reininger.  The school reserves the right to reflect upon tonight’s selections and make adjustments.
