 Employees must be trained to the highest degree.  Small margin for error for each employee.  Employee tasks should be clear to everyone.

 Talk to your employees about safety.  Review all injury reports with employees.  Discuss safety with all employees in an incident, injured or not.  Get everyone involved in the safety process.

 Report all unsafe conditions.  Certify all employees in safety procedures.  Make safety as important as profit.  Hold safety meetings as often as possible.

 Take human error out of operations.  Reduce the domino effect.  Replace outdated or falling apart equipment.
