7  Is your system able to link multiple teachers to a course section?  Is your system capturing and storing (multiple) start and end dates for teacher assignments? Can the end-user view and modify these start and end dates?  Can you preserve grading information after a teacher assignment ends (leave of absence) and restore that information when a teach assignment starts again?  Is your system capturing and storing (multiple) start and end dates for student enrollments? Can the end-user view and modify these start and end dates?  Can your system record period-level attendance?

8  Can your system auto-populate the period-level attendance value with the daily attendance value and allow a period-level attendance override?  Have you finalized your Staff Student Course extract, including ◦ enrollment linkage (duration of teacher assignment – student enrollment linkage)? ◦ attendance linkage extract (duration of teacher assignment – student enrollment – student attendance linkage)? ◦ course duration to the reporting date?

9  Do you have a way to populate the linkage duration adjustment at the teacher-student level with a default value of 1? Can the end-user view and modify these on a student-teacher level to a value between 0 and 1?  Do you have a way for the end user to populate a student exclude flag? Have you implemented security around this flag (e.g., can only be populated by a principal or with the approval of a principal)?  Do you have an interface/summary reports for roster verification at the teacher/principal level, including student-teacher linkage start and end dates?

10  Legislation requires that assessments be used in the teacher evaluation process  Teachers will be evaluated on the following basis: ◦ 20% of evaluation based on state assessments ◦ 20% based on local assessments ◦ 60% based on local observation and evaluation techniques  These three scores will result in an overall evaluation rating on a scale of 1-4.  This template is designed to collect the component scores as well as the final overall evaluation rating.

11  Assessment accommodations will be scanned from the answer sheet used during test administration  For any assessments that do not use a scanable answer sheet, testing accommodations will be recorded in the appropriate student system and exported to the data warehouse using the Assessment Acc Mod Fact template  A list of revised assessment accommodations will become effective beginning with the 2011-12 3-8 ELA and Math assessments during the April/May 2012 time frame  SIRS will be updated to reflect the accommodation revision s
