9 Attendance & absence  Rotation schedule is tied to Preceptor payment and one45 evaluations  You are expected to be present, on time, to scheduled rotations  Any schedule changes need to be sent to your Program Coordinator  Illness? 10 Leave of Absence (LoA)  Absence of 5 days+  Paid medical leave of 1 week+: letter from physician, stating Resident is under medical care & length of medical leave — RTW: requires a letter from physician, stating Resident is cleared  Paid special/compassionate leave of 1 week+: letter from PD stating reason for leave & start and end dates 10

12 Time Off Request  Full year Program: 20 vacation days  Less than full year Program: pro-rated  3 flex days; 1 float day  5 days off at Christmas or New Years (Dec 23-27 or Dec 29-Jan 2) — Alternate religious holiday? Applications are due by Aug 1  Enhanced Skills Attendance & Absence Policy (to be posted on DFM website)  Request submission due 2 blocks prior  One45 demonstration — Resident Procedure (to be posted on DFM website) 12
