AS300/400 year are 3 credit hours. Labs are all 0 credit hours.

Are you taking 12 or more credit hours every Fall & Spring semester Have you included all Summer semesters? You must include Summer semester on the form, even if you are not taking classes Have you printed the form and taken to your academic advisor for review? Did he/she sign Block 4 in blue or black ink?  If you have answered YES to all questions see your academic instructor for review!
