3 Application Process Your responsibility to apply by your school’s deadline! (Typically electronically) 2. On-line applications Completion and neatness 4. Make a copy for your records 5. Many parts of the application

4 Application Essay 1. Most schools require some form of an essay
2. Specific versus general topics 3. Obtain assistance from teacher, counselor, administrator, or parent 4. Give yourself plenty of time to properly write the essay

5 Authorization to Release Pupil Records
This form allows DHS to send official transcripts This form needs to be filled out only once and must be signed by the student and a parent

6 AUTHORIZATION TO RELEASE PUPIL RECORDS
Damascus High School 25921 Ridge Road Damascus, Maryland AUTHORIZATION TO RELEASE PUPIL RECORDS Maryland and federal legislation require that the school must have appropriate written consent for release of student records. This consent is required from the parent or guardian of a student who has not yet reached the age of 18. A student who has attained the age of 18may declare his/her majority and sign for release of his/her records. One release form will suffice for release of pertinent records to all post-secondary education institutions and to prospective employers. No records will be sent anywhere without completion of this form. ***************************************************************************************************************** What is a transcript? It is a copy of your permanent record which shows our final cumulative high school grades and credits, including your unweighted grade point average and your weighted grade point average. Allow three weeks for your transcript to be mailed out – counselors may have a backlog during certain times of the school year, especially if a recommendation needs to be written. Permission is granted for the release of all school records (copies thereof) pertaining to attendance, date of withdrawal or graduation, exact title of courses taken, grades and/or credits earned, individual or group testing, health records, etc. upon request by a college or school. 7 Transcript Request 1. All schools want to see your transcript
2. Transcript Request Form (in packet) 3. Important points on the form: MUST BE FULLY COMPLETED OR RETURNED complete name and address application deadline (DHS requires 3 weeks to process transcripts….see orange form!) first three are free of charge counselor recommendation one form for each school

You can schedule appointments for: Before school After school During lunch periods Be sure to bring ALL materials with you, including: Transcript Release form Authorization to Release Pupil Records High School Counselor Release form (if required by college) Addressed, stamped (3 stamps) 9x12 envelope for each college/school (if the school is not an e-docs school)

11 MC Courses New legislation allows students to receive a reduction in their tuition costs Student must still qualify for MC courses based on GPA and SAT/ACT/Accuplacer scores Please see your counselor if you plan on taking MC courses second semester

12 Step by Step instructions:
Meet with the registrar, Mrs. Carey (3 weeks prior to the college deadline to officially request the transcript) Bring “Transcript Request Form,” “Senior Information Summary” (printed from Naviance), and “Authorization to Release Pupil Records” to Mrs. Carey to schedule a transcript meeting

Your school may also have a report called a Counselor Report/Secondary School Report. If your school has one, it will be located towards the end of the application Please print and bring this form with you (if applicable) when you meet with Mrs. Carey. Bring and envelope with postage (if not sending electronically)

: Request SAT/ACT scores -Complete an on-line request for SAT or ACT score reports to be sent directly to the colleges

15 Step by Step instructions:
Check with colleges four weeks after the submission deadline to ensure that all materials have been received

16 Documents sent to colleges by the registrar:
Transcript, which lists courses, grades, and credits for grades 9,10, and 11, and course titles for grade 12 Counselor recommendation Counselor Form (if applicable) The “School Profile” which describes DHS Mid Year Report, if needed-student makes request in January

17 Final Information 1. See your counselor or Mrs. Carey if unsure of the college application process 2. See Mr. Martin about college searches, Naviance (needed for DHS college visit registration), financial aid, and scholarships 3. Stay organized and watch your deadlines 4. Maintain your grades 5.
