‘The right healthcare, for you, with you, near you’ The Future Landscape The Kings Fund in their 2015 paper “Transforming our health care system” sets out Ten priorities for the system. These have four themes as follows: 1.Systematic Management of Chronic disease 2.Population Based Commissioning 3.Better Integrated models of care 4.Empowerment of patients

‘The right healthcare, for you, with you, near you’ Population Based Commissioning Multi-year capitated contracts that reward providers for delivering outcomes must have: The ability to co-ordinate the care along a pathway and across settings The ability to understand future risks to ill-health and deliver interventions to meet those risks (population health Management)

‘The right healthcare, for you, with you, near you’ Better Integrated models of care Commissioners will need to systematically develop new models of care in response to their own context and opportunities. All approaches will require excellent leadership Leaders must understand population based outcomes and co-ordination of care

‘The right healthcare, for you, with you, near you’ Individual review I understand my own personal triggers, what drives me and what holds me back I understand how others perceive me and am able to modify and adapt my own behaviour when faced with different situations I act as a role model for others and demonstrate integrity in day to day work and interaction with others I demonstrate adaptability and flexibility as required by the situation I am open to feedback from others and actively seek it I maintain insight and focus despite difficulties, am able to admit to and learn from failure and will speak up if I don’t know something I maintain perspective and keep going in difficult circumstances I am able to cope with and balance the pressures of a complex and high profile role I manage conflict well even in personally difficult or potentially compromising situations I have the personal maturity and mental resilience to deal with organisational politics and ambiguity and remain authentic I am able to challenge key stakeholders including the GP membership in the interests of commissioning the best solutions for the population

‘The right healthcare, for you, with you, near you’ Individual review continued I am prepared to give up power/resources for the benefits of the wider healthcare system I take into account a broad range of views in making decisions I am able to step back from the detail to see the bigger picture and the longer term I utilise the skills, insights and expertise from individuals and teams across the organisation to challenge assumptions and develop options I contribute to collaborative planning based on an understanding of how all parts of the organisation interact and in turn how the CCG interacts with the wider health system I understand the accountabilities and roles of all those on the governing body and develop non-judgemental and mutually respectful relationships with my fellow senior leaders (Where appropriate) I am able to distinguish between my role as a GP and my role as a commissioner of services for the whole of Wiltshire
