Who benefits and why Patient with long term conditions: Improved engagement with their general practice team, Greater understanding of their condition, Feel more in control of their health, Have improved health outcomes. Self-Management Support fit Being inspired: information, knowledge, role models and communication Any interaction with a healthcare professional is an opportunity for self-management support Activated patients and activated clinicians

For individuals with early air flow obstruction, quitting smoking may prevent COPD developing If COPD is diagnosed, early interventions can alter disease trajectory and improve outcomes Self-Management Support fit Being inspired- information, knowledge, role models and communication Providing evidence of early lung disease may help provide added motivation and will power to quit smoking. Identifying early COPD means individuals have more control of their self management journey
