2 South Tyneside Lifestyles Physical Activity levels still poor More than 60% do not engage in even moderate exercise 52% in overweight/obese category 30% of 16-44 “obese” (13% increase since 1992)

3 “……introduce a ‘Lifestyle Team’ based on the smoking cessation model to which people can be referred for appropriate support.” Obesity Strategy : Health and Social Care Scrutiny Committee Obesity Commission

4 Aims and Objectives To address health inequalities in South Tyneside. To assist residents of South Tyneside to choose a healthier lifestyle, by offering individually tailored advice and support through a 12wk specifically designed lifestyle programme.

5 The Choosing Health Team The Choosing Health Team is made up of 6 Health and Lifestyle Advisors and 6 Community Health Officers. The Health and Lifestyle Advisors will receive both self referrals and referrals from health professionals to help ‘at risk’ clients change their lifestyle and reducing risk factors. To increase opportunities to access physical activity Improve diet Weight management Stop-smoking signposting Improve physical and emotional well-being

6 One Team, Two Roles, One Outcome Health and Lifestyle Advisor will… Identify the types of interventions that best suit the needs of the individual; Design 12 week lifestyle programmes which will include any of the following; –Health MOTs –Nutrition advice –Exercise options –Weight management –Stop smoking signposting

7 One Team, Two Roles, One Outcome Community Health Officers will… Address and meet the health needs of the community through increasing community capacity Establish community activities Establish and maintain community groups To keep in regular contact with the client over the 12 week period

8 Possible benefits of joining … Lose weight Reduce blood pressure Reduce the risk of heart disease Improve diet Feel more energetic Increase fitness levels Increase confidence Meet new people Improve knowledge and skills with food Make daily activities easier Through these we hope to reduce pressure on GPs

9 Community Bases Each team will have a number of designated community bases throughout each CAF area. This offers more accessibility to the scheme.

12 How to join the programme There are 2 ways to access the programme 1.Referred by Health Professional 2.Self referral.

13 What difference will this make to South Tyneside? Identify, then address the specific health needs of local people in their local areas.
