WHAT’S OLD IS NEW AGAIN with health care and public health in the USA “When I was a boy of 14, my father was so ignorant I could hardly stand to have the old man around. But when I got to be 21, I was astonished at how much the old man had learned in seven years.” MARK TWAIN 1

EXAMPLES OF CHALLENGES THAT ARE OLD BUT ARE NOW NEW OLD MEDICARE TB HIV INFLUENZA SMALL POX POLIO TETANUS RABIES LEGIONELLA NEW AFFORDABLE CARE MDR TB AND XDR TB EBOLA NEW STRAINS OF FLU SARS WEST NILE VIRUS LEGIONELLA (IS BACK) 2

THE PROVISION OF HEALTH CARE HAS REVISED OLD CONCEPTS WITH NEW TECHNOLOGY AND NEW OBJECTIVES DUE TO NEW LEGISLATIVE AND REIMBURSEMENT MECHANISMS OLD HOSPITALS HAD OUTPATIENT AND NEIGHBORHOOD CLINICS TO SERVE THE PUBLIC AND WERE NEARLY 100% NON- PROFIT ORGANIZATIONS PATIENT CARE DOCUMENTATION WAS KEY AND REQUIRED DETAILED RECORD KEEPING AND STORAGE SPACE. RESOURCES AND EXPENSES WERE NOT NECESSARILY PARAMOUNT NEW HOSPITALS ARE BECOMING FOR- PROFIT ORGANIZATIONS AND OPENING OUT-PATIENT AND PRIMARY CARE CLINICS OR SERVICE CENTERS. HOSPITALS NOW HAVE EMR’S TO INSURE EFFICIENCY AND EFFECTIVENESS. COST EFFECTIVENESS IS JUST AS IMPORTANT AS EFFECTIVENESS AND SAFETY OF CARE RENDERED. 3
