Centennial Quality Award Program Both the 2008 Commitment and Achievement are on one form To be used when the commitment is made Used again at the end of the year when the achievement is evaluated The form may be filled in online then stored, and when appropriate, printed and signed

Centennial Quality Award Program The key is an engaged commissioner and committed unit leaders Each commissioner must work with the key unit leaders to set realistic, achievable, goals to improve their program

Centennial Quality Award Program Unit Action Planning Meeting This is an opportunity for an honest unit self-assessment Useful tools include: Data regarding past achievement 2008 Centennial Quality Unit Award Commitment and Achievement Form Unit Self Assessment forms and spreadsheet

Centennial Quality Award Program Expected outcomes of the Unit Action Planning Meeting The self-assessment will identify unit strengths It will also identify areas to improve upon An opportunity to establish the units’ vision for the coming year Determine specific steps needed for the unit to improve Schedule all follow up necessary to monitor progress

Centennial Quality Award Program Commissioners follow up with the unit Provide regular support throughout the year Monthly unit visits are essential; more frequently if needed Refer to tools used during the Unit Action Planning Meeting 2008 Centennial Quality Unit Award Commitment and Achievement Form Unit Self Assessment Spreadsheet
