Promote, Protect and Heal Promouvoir, protéger et guérir What is Case Management Popular means to coordinate patient care and control resources. A process model that provides a mechanism for managing cases across the continuum of care, thereby providing a seamless, integrated care process.

Promote, Protect and Heal Promouvoir, protéger et guérir The CM program will provide standardized services to eligible CF members based upon a population health focus that is: Client centred Seamless Respectful Timely & appropriate Effective & efficient

Promote, Protect and Heal Promouvoir, protéger et guérir Our Purpose To help members : Get the care they need, when they need it Become more knowledgeable about their medical condition and benefits for which they may be eligible Achieve continuity of care Make a smooth transition back to duty/work or to civilian life

CF members referred by an attending clinician with complex needs including acute health care requiring coordination of services likely to exceed 3 months from time of onset; CF member who is medically releasing (3a/3b); and 5F deemed disabled; CF member referred by an attending clinician for Complex Return to Work;

CF member assigned a Permanent Medical Category with Medical Employment Limitations by D Med Pol; CF member on period of retention and/or posted to JPSU (Service Personnel Holding List (SPHL) (screening required), and CF Member referred for a second extension (>1 year) of a Temporary Medical Category.

Help members and families navigate the military and civilian healthcare systems Provide information and contact points Inform members about when and how to apply for benefits and services Help members coordinate their medical care
