Describe the process that we have undertaken in Fife to develop structured patient education for people with diabetes November 2006 –raised at MCN board meeting

Patient education activities in Fife Dunfermline monthly group education West Fife patient education programme Acute Insulin conversion group education (Fife DICE) Patient focus sub group meeting Diabetes UK patient meetings and conferences

MCN sub group – who, what and how? Fife wide, multidisciplinary & patient involvement, with support from SDG & Diabetes UK. Considered available programmes for Type 1 and Type 2 diabetes, also children. Spoke with and/or arranged visits to centres delivering programmes. Completed SWOT analysis based on information gathered.

Developing a proposal – delivery model

Developing a proposal Agreed to recommend DAFNE for type 1 programme & X-pert for type 2 diabetes. Develop model of educator teams Identify requirements for delivery e.g. admin support, venues. Discussed this with CHP’s to reflect local differences particularly for resource options.

Business case Tabled at Balance of Care Group – December 2008 Discussed with CHP & acute division senior managers Used to make application for SDG funding to establish DAFNE Looked for funding from external organisations e.g. LTC collaborative

Raising awareness of proposal Presentations at CHP meetings and protected learning session. Diabetes UK patient events. Workshop at MCN conference

Where now? Letter to chairman of NHS Fife Re- establish MCN sub group with aim of reviewing current provision, updating previous proposal & identifying administrative model.
