Part 4-1: Other Statement of Endeavors (Clinical Activities) (for I-M faculty only ) Clinical Activities should be anything related to your clinical practice and working with patients, patient care, quality assurance, etc.

Clinical Activities Statement of Endeavors 1. Administrative Responsibilities: (administrative history of your clinical activities)  Positions, roles in clinical care  Leadership positions

Clinical Activities Statement of Endeavors 2. Service Provider: (patient/clinical care)  Explain the type of clinical service you provide and what your clinical expertise is in, why is it significant  Provide description of and evaluations of quality of the service and care you provide (awards, patient evaluations, quality assurance efforts)

Part 4-2: Supporting Materials OTHER: CLINICAL (for I-M faculty only) Examples:  Patient satisfaction surveys (competence in professionalism, communication, humanism)  Thank you letters from patients, families  Recognition (awards for clinical or educational excellence)  Participation or leading of clinical quality improvement initiatives  Administration of clinical and/or educational training programs  Development of new clinical services  Serve as specialty resource in area of expertise
