Ohio Access Success Project

Assisting individuals who live in nursing homes to move to an independent setting What Is The Success Project?

What Does The Success Project Include? The Success Project will include limited funding to pay for one time relocation costs.

Who Is Eligible?  Your cost of care in the community is less than the cost of care in the nursing home  You are Medicaid eligible AND  You want to leave the nursing home and live independently We May Be Able To Help

How Can We Help? Step 1: A member of the relocation team will meet with you for a personal interview. Step 2: You and the team will determine your needs, strengths, community supports and issues that enable or prevent you from moving to an independent setting. Step 3: If you decide you want to move - you and the team will develop a plan.

How Can We Help Cont. Step 4: You can choose not to continue in the process at any time. Step 5: Relocation resources will be available to you to help with moving expenses. Step 6: Inform you of what we can’t do for you.

Who Are The Team Members?  YOU  Social Worker, Team Leader  RN  Peer Coach  Your Friends and/or  Your Family Members

What Is A Peer Coach? A Peer Coach is someone who will support you and assist you, if needed, during the plan.

What Is Your Job? Be part of the plan--- This is your life, you need to decide how you want to live it

Who Will Help You? YOU Peer Coach Nursing Facility Staff Friends Social Worker Team Leader FamilyRN

How Can You Move To The Community? Team members will identify what you need to be successful in the community.  Personal Care  Meals  Housekeeping  Housing  Transportation  Emergency Response Systems

What Happens After You Move To The Community? Team members will follow-up with you after you have moved to the community.  1 week  1 month  6 months  1 year

Are You Interested? What’s Next? Review the information you heard today. Ask questions. Let us know if you want more information. Tell us if you want to move to the community. Remember you can change your mind at any time.

How Do You Contact Project Staff? You may contact Project Staff at any time for additional comments, questions, or if you or someone you know is interested in moving to the community from a nursing home. Ohio Access Success Project Staff:
