Early Intervention Parent Support Plan Mission of First Steps Georgia Goals: To provide expectant parents and families with children ages birth to five 1) current information on topics of interest and relevance to the family; 2) information on community resources relevant to the family; 3) services to strengthen the protective factor of concrete connections to promote maternal and infant health for expectant parents and parents of children birth to five. Planning and Implementation: With community partners, develop and implement a First Steps Georgia plan to provide universal, early intervention services and support for families: 1.Target Population/Service Delivery Location 2.Access 3.Identifying Needs/Community Resources 4.Documentation 5.Coordination First Steps Georgia Summary 5

Services and Support- Just as every family is different, every community is unique and the design of these plans for supporting families will be different. The proposal should be tailored to the specific needs of the community. At a minimum, the following functions (or services) are required: 1. Initial contact 2. Linkages to community resources 3. Educational materials relevant to families from each of the required topic areas 4. Mandated materials 5. Community resource guide 6. Parent telephone number 7. Contact information 8. Applicants must:  Be a public government entity or 501(c)(3) non-profit  Serve as the fiscal agent for the grant and the point of contact to GOCF or Provide a MOU with fiscal agent  Be able to submit timely and accurate program and financial reports  Be responsible, liable, and oversee financial, program and post- award reporting requirements  Plan or continue to provide universal, early intervention supports and services in accordance with the First Steps Georgia Plan. Deadline for Proposal Submission: Proposal cannot be hand delivered and must be mailed via postmark by August 9, 2013 Governor’s Office for Children and Families Child Abuse and Neglect Prevention Program 7 Martin Luther King Jr. Drive, Suite 351 Atlanta, Georgia Grant Award Period: 12 months, October 1, 2013-September 30, 2014 – All service delivery activities (new, enhanced or expanded services) must begin within 90 days of the start date of grant award. An eligible applicant must be applying to implement or continue universal, early intervention support and services in accordance with the First Steps Georgia plan. Each applicant is eligible to apply for a 12 month “mini- grant” for services and supports that meet the criteria of First Steps Georgia and is a start-up or continuation of an existing First Steps grant award. Applicant may apply for funding in the amount of $6,000 or $12,000 conditional upon the project number of additional families that will be served annually: $6, additional families $12, additional families Grant award period: October 1, 2013-September 30,2014 Grant Award Amounts 11

The granting of funds in one year is not a commitment to continue assistance in the future and does not guarantee future funding. If there is continuation funding, it will be contingent upon: 1.Organizational capacity; 2.Performance history and grant compliance; and 3.Availability of GOCF funds Continuation Funding 12

Applicant is requesting funds to support First Steps in a county where First Steps is currently receiving funds through a Maternal, Infant and Early Childhood Home Visiting (MIECHV) grant; Submission fails to be postmarked by U.S Postal Service or commercial carrier by deadline date; Submission fails to include the original signature of the executive officer of the fiscal agent (applicant) on the application face sheet; Submission fails to include the application program narrative, application budget and application attachments; Application lacks original signatures, where requested. The screening process for First Steps Georgia (once families have been referred to this service) must be the Great Start Georgia Core Screening Tool. Target populations and service delivery location(s) – 2. How will the program ensure parents of all ages and all ethnic and socio-economic backgrounds have access to First Steps – 3. Initial Contact: Describe First Steps initial family contact in your community by providing a detailed response to each of the following: – How will the initial contact be made – Relevant and age-appropriate materials and information – Describe the localized, annually updated Community Resource Guide – How will the program provide contact information to reconnect with the First Steps staff? – Provide a description of how the program will provide families with a parent support telephone number, available 24/7 if possible. Total Wages Earned Monthly – This should reflect a person’s annual salary divided by 12 months. – If the employee is hourly, multiply his or her hourly rate by the number of hours they are anticipated to work in a year and divide that number by 12 months. $8 *2080 hours (52 weeks*40/hr per week) 12 Personal Services

Taxes and Benefits – This should be the amount of taxes and benefits the employer pays on behalf of the employee. – If your agency uses a fringe rate, multiply this by the employee’s annual salary and divide by 11. – DO NOT include the amount of taxes that are removed from an employee’s paycheck, these are already accounted for in his or her salary. % FTE – This is the amount of time an employee spends working on the GOCF project. – Example: Sally works 30 hours per week for the school system. If on average she spend 15 hours per week on the GOCF program, her FTE should be (.5). If on average she spends 30 hours per week on the GOCF program, her FTE should be (1). Personal Services

Number of Months – This should reflect the number of months an employee will work on a project. – If he or she only works 10 months out of the year on the GOCF project, then this is the number of months that should be applied to the grant. Personal Services
