2. As the Central New York Care Collaborative (CNYCC) moves from planning phase to project implementation, we would like to formally announce the kick-off of the DSRIP Year 1 Contracting Process. The contracting process has been developed to ensure timely and accurate payment procedures for CNYCC partner organizations. Contract completion is an important step in participation in the CNYCC PPS and eligibility for DSRIP payments. CNYCC partner organizations must complete a four-phase process in order to be eligible to receive re-imbursement payments including: Standard Partner Contract, Business Associate Agreement(BAA), Project Selection and Organizational Information Survey, Contract Addendum. CNYCC Partner Organization Contract Process Phase NumberContracting PhasePhase DescriptionCompletion Date 1 Standard Contract The Standard Partner contract includes all the basic components of contracting with CNYCC to participate in the PPS. This contract is considered the core contract. Specifics around payment and project-based responsibilities will be included in Contract Addenda entered into between CNYCC and partners. Friday November 6 2 Business Associate Agreement (BAA) The BAA is required to cover the exchange of data and information between your organization and the CNYCC. Friday November 13 3 Project Selection and Organizational Survey Each partner will complete an electronic survey which confirms/recommits them to specific CNYCC project participation. Project commitment is extremely important as it drives the PPS’s ability to achieve project outcomes, accountability, speed and scale of project implementation and other important factors. In addition, your organization’s respective partner contract addenda cannot be issued until project selection has been completed by your organization. Lastly, the partner survey will solidify points of contact partner organizations. Accurate contact information is a vital component in building relationships with partners and implementing DSRIP. Friday, November 20 4 Partner Specific Contract Addendum The Contract Addenda will provide a detailed outline of roles and responsibilities your organization will perform in the specific projects they have chosen. Contract addenda will include specifics on each project partners have selected, participation requirements, and payment flow, and reporting /performance requirements. Friday December 4
