Self-assessment Using Quality Indicators (QuIST) The QuIST is multi-dimensional in scope and designed to: Facilitate communication and sharing within and among the district and its interagency partners; Identify areas of strength and opportunities for improvement; Promote planning & continuous improvement; Evaluate and measure progress over time.

********** Online Data Collection System Log In: Type your address and password then click on the ‘Log In’ button Contact CCTS for a district account and password. Then enter it under the ‘Log In ‘and click on the ‘Log In’ button.

Starting a Building QuIST Assessment Form a Building QuIST Team (4-12 members), read the directions and click on ‘Add Building QuIST’ To download a hard copy of each domain, click here. Click here to begin the QuIST
Getting Started To download a hard copy of each domain, click here. Click ‘NEXT’ to enter ‘Building Data’ or scroll across the tabs at the top of the page to access other sections of the QuIST.
This will take you back to your district’s QuIST home page where you may add or delete building QuIST assessments Click ‘Submit QuIST’ only after rating all 100 indicators for your building.
School-Based Activities The team will determine the frequency to which each indicators occurs and provide only one team response for each indicator.
OSPI Evaluation & IEP Technical Assistance Module 2009 School-Based Activities Rate each indicator for every sub-domain and click ‘NEXT’ to continue with the next domain on the following page.
Work-Based Activities Repeat the evaluation process as a team for each indicator across all five domains.
OSPI Evaluation & IEP Technical Assistance Module 2009 Connecting Activities Once the team has rated all 100 indicators, click ‘Submit QuIST’ If you omitted a response, you will receive an error message with specific items left blank.
Submitting a QuIST and Generating a Report After responding to any error message regarding omitted responses, click on ‘Submit QuIST’ and this page will appear. Scroll to the ‘Reports’ tab at the top of the page to review the building QuIST summary scores and downloadable report.
