Requirements 1.Microsoft Explorer 6.0 Mozilla Firefox and Safari web browsers on Windows XP, Vista, and MAC Operating systems. 2.Download the latest version Adobe Acrobat Reader which is free. 3.All users automatically see the latest version of the application as soon as it has been deployed by the Minnesota Department of Health (MDH) Office of the State Registrar (OSR). 4.You will, however, have to login to the system with a username and password.

Login Screen/Self Password Reset 1. Access the test URL site 2. Enter Your User Name 3. Enter Your Password 4. Select the Login Button 5. Reset Your Own Password if you forget.
