Our achievements 04/05 1. Primary health care services for the community Continued development of statewide initiatives to improve support for families and young children’s health: Universal Home Visiting Family Home Visiting Universal Hearing Screening. Development of new or improved programs in communities: Thebarton Maternal Outreach Service The Corner House. CAFÉ Enfield.

Our achievements 04/05 1. Primary health care services for the community Commenced lead role implementing health’s responsibilities within the Keeping Them Safe reform: Extension of therapy programs for children who have suffered abuse Increased counseling services for young people who have been sexually abused. Continued development of Health promotion programs: Nutrition with schools and pre-schools The Human Race in northern suburbs Obesity research in pre-schools children.

Our achievements 04/05 Our Strategic Plan seeks to strengthen Primary Health Care whilst maintaining the viability of tertiary care services. Four of the six strategic goals will advance a Primary Health Care approach: Contribute to the population’s health and wellbeing Improve opportunities to prevent illness and promote health Improve the health of Aboriginal children, young people and women Address health inequalities. 2. Planning and policy to support Primary health care Developed a five year regional Aboriginal Health Strategy with key priorities in: Community involvement and partnerships Workforce development Access to health services Targeted health improvement programs. Commenced leading the development of the Women’s Health Action Plan to implement the South Australian Women’s Health Policy. Developed our draft community engagement framework.

Building our capacity for Primary Health Care Established Board Subcommittees for Aboriginal Health and Population Health Appointed Executive Director, Population and Primary Health Care General Manager, Health Services and Executive Director, Strategic Development and Management also have key support roles in Primary Health Care Developed discussion papers to stimulate organisational thinking about Primary Health Care and Population Health

1.Develop a comprehensive strategy in one or more communities to improve birthing outcomes for Aboriginal mothers and infants 2.Develop a community based, population approach to promote healthy weight for children 3.Contribute to the development of Children and Family Centres at Cowandilla and the Parks 4.Contribute to planning of integrated, accessible primary health care services at Elizabeth, Marion and Aldinga 5.Develop the SA Women’s Health Action Plan in partnership with others PHC Priorities for
