6 Draft National framework for joint state/Commonwealth plans Consistent with National primary health care strategy 4 draft strategic outcomes: Build a consumer-focused integrated primary health care system Improve access and reducing inequity Increase the focus on screening, prevention and early intervention Improve quality, safety, performance and accountability

7 Work to strengthen CHSs Community Health Indicators Drawn from the Commission's work on indicators for primary care and developed for CHSs – some have a focus on SC and ICDM Progress to date – draft suite of process / structure indicators developed, recommendations for outcomes indicators being considered Community Health Integrated Program Guidelines Service design based on local population needs Improved consistency in service delivery and program design Increased implementation of evidence based practice Focus on client flow in accordance with service coordination principles

8 PCPs reporting 2011 - results Highlights: All PCPs have core agencies as members, and increasingly are engaging with mental health services and human service organisations The SC survey indicated areas of consistent practice include; obtaining client consent for disclosure of information, acknowledgement of urgent and routine referrals, implementation of SC principles into practice The ICDM survey indicated that the following improvements were made; an increase in consistent assessment, improved provision of self management Some case studies flagged strong consumer engagement in PCP work Some case studies showcased IHP and ICDM work that led to tangible organisational improvements at a catchment level (that are expected to result in better care for clients)

9 PCPs reporting 2011 - results Areas for improvement: Reporting identified that consumer engagement could be strengthened The SC and ICDM surveys indicated that areas that require continued focus are; coordinated care plans and sharing care planning information with General Practice Very few case studies identified the benefits for providers and for consumers Few ICDM case studies referenced the development and implementation of care pathways for clients with chronic/complex conditions Many IHP case studies lacked detail about evaluation results, and the impact of work
